    Accidents are caused by (unsafe acts or unsafe conditions): Incorrect use and abuse of plant and equipment. Use of faulty equipment and “homemade” repairs. Illegal adaptions and illegal removal of guards/barriers. Failure to use PPE and ignoring safety signs/warning devices.
    Some examples of unsafe conditions are: Poor guarding - inadequate or inefficient. Defective Conditions - hand tools, equipment, substances. Poor Layout - work flow, overcrowding and congestion.
    Unsafe conditions are hazards that have the potential to cause injury or death to an employee. Unsafe conditions can be found in a variety of workplaces, but they pose a special hazard to workers in industrial, manufacturing, or manual labor positions.
    Other examples of unsafe acts include disregarding posted warning signs, failure to wear a hard hat, smoking near flammables or explosives, working too close to power lines, handling chemicals or other hazardous materials improperly, putting your body or any part of it onto or into shafts or openings and lifting.
    Unsafe Act can be defined as any activity by workers which are not as per the prescribed safety standard or practice and which can cause or likely to cause accidents or risk for self or others at workplace, damage equipment and bring losses in terms of reputations and revenue to employer.
