    Today's Video:
    The T-L-I-F is defined as Transforaminal Lumbar Interbody Fusion. This approach requires a small incision on the
    back and can be done as a minimally invasive procedure through a 1 inch portal.
    Initial supplemental posterior fixation is used to considerably enhance the stability of the motion segment.
    The traditional T-L-I-F approach requires that the facet joint be removed to access the spine.
    This allows the implant to be delivered without manipulating the spinal cord.
    The surgeon can then remove the diseased disc nucleus to prepare the vertebral bodies for fusion.
    First bone graft is placed in anterior disc space.
    After the disc is prepared, a biocompatible polymer implant is filled with bone graft material and placed in the
    disc space.
    Additional bone graft may be placed around the implant to increase the surface area for fusion.
    This procedure is normally accompanied by a posterior fixation with pedicle screws or facet screws
    The implant maintains the spacing of the vertebrae while the fusion takes place.
    After the fusion is completed and a solid bone mass forms; the two vertebrae are joined together.

    Did my TLIF last week and I already don’t feel those crazy nerve pain and hoping to be fit for my kidos family and close friends . It’s a pain procedure but worth it is done by an expert surgeon.Ya shafee❤

    This is exactly what I will have done in June. I am looking forward to not having pain like I have experienced for the past 4 - 5 years. Healing of the surgical area I know will be painful but that heals in time. This is an excellent video explaining the surgery and the workings of the entire vertebra system.

      I have done same
      Unfortunately still have pain and stiffness lower back.
      Even physiotherapy exercise not working 😢😢
      I should not have done.. 😩

    I had my lower back surgery decompression and fixation sometime in 2012 and have been fine Since then except for occasional numbness at my feet. I complement the treatment with constant workouts and other back stretching exercises including use of back support at work. The Iranian Hospital at Dubai.

      I had surgery on December the 2nd of 21. Yeah i have muscle spasms and pain. But i have diabetes and it taking long for the wound to heal.

    What long term risks are there?
    The German disc replacement surgery has always been popular because these fusions had a high risk of major long term issues in the nearest disc eventually?

    Thank you for these videos. I had a TLIF in 09/20 due to issues at L4-5 on the right side. No left side issue at the time. However, now I am having issues with the L5 on the left side and am having a Laminal Foraminotomy. My surgeon said he would to remove hardward in order to get to the L5 on the left side and that it would be an outpatient procedure. So, this was very confusing considering the initial TLIF. This video was really helpful for me to under better about the instrumentation and placement.

    Hello, thank you for a great video clip. One thing is.. the caption in the right upper side is incorrect (it says 'PLIF procedure') from 4:06 while audio continuously explains TLIF procedure.

      @@DhileepR No , procedures slightly different. PLIF makes incision down the mid back , TFIL 2 incisions on either side of midline spine. it avoids cutting nerves

    After the two vertebrae are joined together (fully healed) - are the pedicle screws removed in a subsequent procedure - since the support they initially provided seems no longer necessary.

      Some people do go in and have the hardware removed, but usually only if it’s causing issues for them. I had my fusion when I was 10 and I’m turning 35 this year and still have all of my hardware in place. I don’t feel like it’s worth it to go in and get rid of it when it’s not bothering me.

    Once the bone fuses you are much less flexible in that region right?
    Is this a good thing?
    What happens if you over stretch that region? It looks like something potentially painful

    3:27 that`s exactly what i have, but does this mean the disk is samaged and must be replaced? or can it heal back?

      It does not have to be replaced. The small piece that is "slipped" can be removed and the rest of disc can be left alone. Speak to your doctor and see what he/she recommends and feels is best for you.

    Dr. Ara Deukmedjian, a neurosurgeon at the Deuk Spine Center in Melbourne Florida (just southeast of Orlando) has a orthoscopic (very tiny cut) approach (fast heal same day outpatient) without the need of fusion or its hardware and long healing time. Dr. Deuk can heal bad discs with a patented laser approach. Having had a friggen fusion, (leading to decades of more damn costly revisions due to adjacent disc damage over time), I would not personally not choose this aggressive approach again. The US has been so far behind. It surgeon’s like Dr. Deuk and Tony Mork that use tiny incisions to perform back surgery. Everybody needs to make their own decisions, I get that. It’s a question if you have your MRI reviewed by these advanced surgeons. The tiny inconvenience of travel has never stopped me from finding the best one out there. For some the fusion may make sense.

    I am having this surgery in a week. Have been suffering extreme pain in left hip and all down left leg and foot due to a pinched nerve no one can find!!!! Sure hope surgery helps..

      have your pain doctor , surgeon perform nerve conduction studies to see where pain is coming from. Don't have surgery without knowing where pain is coming from. Good Luck I hope you find the source. You might have to do mylogram as well.

    I Have had three neck surgeries,1st was a failed fusion and screws backed out eventually broke.After 2nd fusion was in minor car accident screws broke once again. 3rd surgery an artificial disc placed in C-2 and C-7 fused at C-5-6. I now can move my head side to side up and down. The surgeon told me he believes and I truly believe that your spine is free motion . Where is the free motion with rods in your back? I now have problems at T-10 down to S-1 , L4-5 S-1 being priority now what? Searching the U S for new technique other than fusion and rods

    I've had a second herniation and discectomy on my l4 l5 disk. I'm mortified of arthritis and potentially having to get a fusion in the future. Wtf fml
    Also, I feel like in 100 years humans will look back at the TLIF as barbaric 😭.
    Orthopedics seems to be carpenter for the human body 😭🥱

    I had a L4-5 and L5-s1 fusion in the beginning of 2017 after 2 years of TERRIBLE back pain/Leg pain (herniated disc)and then at the end of the year I had the screws and rods removed. (Still had pain, different pain dr said should remove hardware) Since the removal I’ve been so much worse! MRI and X-ray shows “perfectly healed spine” can anyone help ? Where could this pain come from? Can’t sleep, EVERYTHING HURTS! back breaking pain when cough, sneeze, try to run. I’m only 31 but I feel like I’m 90!

      @@nimanajeha1564 back still hurts, less but hurts. Still some numbing and tingling all the way down into my toes. Sneezing still sucks and Yeah, I’m still hurt. Less? Maybe, but from when?!?!? I’m not even sure! 😟

    Do not EVER do this unless you are put in a position of no choice. Hands-down the most painful operation I’ve ever had and I’m not fusing so it’s back to Mayo for another op. Beyond upset.

      So sorry that it’s not going well for you. I had three surgeries, including a fusion, when I was 10. Thankfully mine fused and healed up fine, but I have heard of other people having problems.
