I did it once in anterior tooth of a patient, because root was resorved due to pathology for long time and access opening was done by some other doctor so i was not aware about that.

I do not use sodium hypo chloride saline may be not so much effective but at least no accodent
Without sodium hypochlorite no root canal treatment
