On my first visit to Ghana in 2015 I was not prepared to fill this form and did not have the address of my friend who was meeting me (she was waiting outside). The immigration officer sent me to the head officer who called my friend who told him where we would be staying.
Never mess with immigration officers as they can seriously spoil your journey. I’ve always been prepared on subsequent visits and had no problem

Please you would need to fill the emigration green paper (this type of green card paper is found at the airport) form and you will need to show your PCR test results at the airport. The PCR test must be done at the airport car park on a day before traveling to Ghana.
