She shouldn't have to vacate the title. If Roman could defend his title every 6 months, and Seth could keep his title when he was injured before mania then Rhea should be able to keep her title.

It’s that if a title doesn’t have to be defended for months then she should be able to keep the title, regardless of the reason. It’s about the precedent, not about Roman’s leukimia. As a man who has watched Stardom since 2013, I am well acquainted with AZM. I remember when she was a snot-nosed kid wearing camo pants calling the other ladies grandmas... Aah, the memories. The point is that AZM is AWESOME and I'm really happy that the rest of the world finally got to see that. As an aside, can we get my beloved Black Peach Momo Watanabe (not to be confused with Momo Kohgo) on AEW TV please, pretty please? If you think AZM is great, which she is, wait until you see Momo, she'll blow you socks off.

People can get injured from much smaller things. There's a lot of luck involved
They should just do what they did in 2005 with Trish and just have Rhea stay champion. Shes the most over woman in the entire company and probably Top 5 full timers male or female. Just have her as an on screen character and use the storyline that no one can beat her until a new star emerges from NXT or free agency.
Just take it off her and have someone else keep it till she's back

I say give Liv Morgan the title and turn her heel. Have her go the Drew McIntyre rout and take credit for the injury and she'll get so much heat from the crowd.
Possible Hot Take: HHH isn't a good booker for the woman's division.
He's had a year to work with them and both Iyo and Rhea's title runs were not memorable. There were also almost no woman's division feuds or storylines in the last year that were compelling at least compared to the mens division main belts or even both men's mid card belts.
The best thing going in the woman's division was Rhea but not because of her title run. HHH does get props for his direction in her character work. He's not failing at everything in that division but its clear its a weak point for him compared to others.
Have a safe recovery Rhea. Hopefully you return as the proper top face of the division (and possibly entire company) that fans have wanted you to be for ages now.

Seth rollins didnt wrestle for months when he was injured. Roman defenda the title every 3 or 4 months. And now it looks like cody is on that wagon, just comes out to do promos, guess he just wrestles on ppvs now only. Not a fighting champion
