    I dont understand. The right eye gives a left image and thr left eye the right image. But in your example its vice versa

      It is as I mentioned in the video itself third nerve palsy with intorsion tilt

      @@InsightOphthalmology there is superior and inferior rectus palsy so the image shouldnt be tilted right? they dont cancel each other?

      as we are dealing with third nerve palsy, the inferior oblique is also affected ( responsible for extorsion!) . However the superior oblique is intact supplied by 4rth nerve ( responsible for intortion) . Therefore we see an intortion tilt . :)
