Low Grade Fever - Causes and Diagnostic Approach

  • Low-grade fever (LGF) is defined as a body temperature between 37.5 degree Celsius or (99.5° Fahrenheit) and 38.3 degree Celsius or 100.3°F Fahrenheit (38.3°C).
    This temperature is below classical value reported for fever of unknow origin, but higher then normal temperature.
    If low grade fever has defined cause, then we treat underlying disease.
    If we can not define cause, then we act like in case of Fever of unknown origin.
    In case of Low grade fever, main focus is on Infectious diseases. Because in almost 60 % of cases it is caused by infections.
    Only 3 % of cases are neoplasm and malignancy. Around 6 % is non-infectious inflammatory disease.
    18 % is miscellaneous causes. And 12 % is undiagnosed.
    Low grade fever also can be presented as atypical symptom of covid 19.
    Especially on recovery period which is called convalescence. At this cases low grade fever can lasts for several days during convalescence period.
    Low grade fever can have non specific signs and symptoms:

    Warm skin, A flushed face, Glassy eyes, Chills or Shivering, Sweating, Headache and Muscle Aches.
    Diagnosis of fever of unknown origin:
    First Step is Complete history and physical assessment. if we can define cause we can follow our findings.
    If We can not identify underlying cause, then do tests:
    Complete blood count, liver function, Blood Culture, Erythrocyte sedimentation rate, Urine analysis, Urine culture, Mantoux test for tuberculosis, and Chest Radiograph.
    If we still can not identified underlying cause then,
    CT scan of abdomen and pelvis, with contrast.
    Then Think accordingly.
    If malignancies is suspected then, biopsy,
    If autoimmune disease is suspected, define rheumatoid factor, ANA.
    PPD or Mantoux test should be done in all patients with fever of unknown origin. Its inexpensive and less invasive test. Which has high informative value.

    Low grade fevers since Feb after influenza B here. I've had all these tests and then some w/integrative medicine, immunology, rheumatology, ENT.... EBV recurrence seems to be the only explanation for me. Stay strong whoever is struggling with the same thing .💌

    I am 19 years old. 3 months ago I got a bad flu which left me feeling tired and brain foggy today. The temperature may rise to 37.4. I was told that maybe my body is fighting the herpes virus. Then I took tests, everything was normal except for lymphocytes of 63% and low granulocytes. Now I had an MRI of the brain (they found cysts on both sides), a test for thyroid hormones, and fluorography for diabetes mellitus. Everything was clean, I re-tested the usual blood test and everything is also normal except for lymphocytes 47%, the symptoms persist. Today my throat started to hurt and my voice was changing. The day before yesterday I sang a lot and smoked electronic cigarettes, I smoked and before that I drank very hot teas, and yesterday I also smoked an electronic cigarette. Today these throat symptoms started without smoking. What happened with me. Can I have throat cancer at 19? And what's wrong with me anyway?

    I have permanent low grade fever and sometimes fever for 3 years now, between 37,5 and 38,3. I feel ill and exhausted all the time. I have brain fog and my head is too warm. Sometimes I have a high heartrate The doctors are not taking me seriously. They did some blood tests, couldn't find anything, and said to just rest a bit. It has been 3 years. I can not function like this. How do I find medical help? What do I have? The ortomolucular doctor said it is leaky gut syndrome, but their treatment does not work either. What can I do? Does anybody recognize this?

      @@coachpramilagupta Thank you for your suggestion, every idea is helpful. And yes, I have. I am trying to get a referral to a specialist in autoimmune disease. But the doctors keep saying it is probably stress. But I have these same symptoms, not matter what, stressed or not stressed so... it makes no sense. If I had an idea of what it could reasonably be, they might consider referring me.

      @@coachpramilagupta What lifestyle changes? Do you have any suggestions? I did start a strict diet for leaky gut syndrome, along with some probiotica. I also stopped exercising because I just don't have the energy anymore, which is bad, but I feel worse after I do, or when I do too much in general. I had moments where I couldn't walk from my living room to the kitchen and back without having to sit down. Now I can do grocery shopping without having to sit down. The progress stopped after that. But working only 3 days a week is extremely difficult. I just come home and go to sleep. The other 4 days I am trying to recouperate. I don't do anything else anymore even though I really want to. I am just so tired, and my head is too warm and fuzzy to do my job well. What lifestyle changes could I make? Do you think I should try exercising again?

      Some more information: Some people suggested I might have lung covid, but I haven't had covid. Some other people suggested lyme, but that is extremely difficult to pinpoint. After that, I remembered that I was bitten by a tick when I was about 6 years old, but that was 25 years ago... When I had antibiotics for another unrelated issue, my symptoms disappeared completely for about 3 weeks. Then they came back. That is an importand clue, I guess. Also, my temperature before all this, used to be 36,8 around degrees. Not between 37,5 and 38,3. But the docter says it is not a fever and not a cause for concern.. but is is all the time.

      @@salmanulfaris734 5 months now.. still elevated temperature 36.9 -37.2.. feverish but no fever .. i read somewhere that unexplained low fevers are associated with longcovid..
