    Great video. Thank you for making it. I have degenerative disc decease, and ruptured disc L2-L3. After 3 years of not really treating symptoms, I finally sought some help from a new neck and spine doctor. It would seem that the ruptured disc was not even in play in my case. It is indeed facet syndrome, and arthritis, in facet joints, mainly on the left side. I'm happy there is a new target to look at. Thank you for mentioning enhancing flexibility in neighboring joints, to aid. Stretches for my condition now make a LOT more sense to me.

      My mid back seems to be locked and there is no flexibility and very limited motion. Can this be a facet syndrome?

      I can't do moves like child's pose because of injuries that I sustained during a car accident in 2013, resulting in broken femurs. As a result I had rods inserted to fix my fractures.

    Love this, I explain this to my massage students. " if something isn't moving correctly, there is often pain above, or below or beside. release the hypertonic muscles and then the joint should move correctly again"! Thanks for this great video!

      And at times encouraging that movement can bring on excruciating pain, the brain has organised the body in a specific way of an adaptive measure to restrict movement, for good reason.

    Priceless information to someone in pain. Squatting while rounding the spine isn’t often included for facet joint pain but it was the only way I could get mobility the affected facets.

    Oh man this makes so much sense. I don't know how long I've had lumbar facet but it was diagnosed in Jan. this year and it's been hard to figure out what exercises to continue or discontinue. I've been doing all types of workouts consistently since age 14 and personal training for 22 years, plus performing in heels with various types of props since age 3 (performing way more in my 40s then when younger). I do tons of core stability work, pilates and advanced yoga but nothing seems to alleviate it for long lately. I am now able to keep my back from constantly going out like it was the past 3 years but I think the info in your video is really going to help to get more stability without having to train more. Thank you!

    Can also be a consequence of DDD, when the disk shrink the facets are close from each others and cause pain.

    Thank you doc. Adam. It was very interesting. I have been experiencing pain in my lower back for the last 7 years ( I am 30 years old ). After many visits to physiotherapists, neurologist, etc and after changing over 10 times the matress, the conclusion seems to be that I am having issues with the facet joints and that is why I came across with this video. Do you have any suggestions about exercises that could relief this pain? Also do you think the matress I am using could be the reason for this pain?

    Awesome videos thanks. Everyone always shows videos for this problem at the lower back mine as you mentioned is in my neck. Causes insane pressure up my head and into my thoracic. I’m assuming it’s c7-t1 area since my c7 amd t1 is rotated. I’m in Canada. Can you please provide a video on what to do at home if you have it in the neck ? Thanks

    wow, thxx so much! I have OA and my Phys Med Dr said no way to "hi-velocity" adjustments. I can almost feel what you are saying.. and the relief that would come from this...however I did almost jump out of my skin when you put your fingers on the demo spine and "opened the can"!
    what to do for my facet degen, unable to bend, with one hip higher than the other?

      @@AdamJStoryDC I have two mild Facet diseases mild nerve pain, 3 2.5 mm herniated discs and mild canal stenosis the Dr says it's mild I don't need surgery, chiro and PT say never get surgery they can help me live pain free is this true I won't need surgery and can get adjustments please help im so worried

    Hi Doc,
    I got DDD at L4/L5 with minor annular tear, very little hernia and facet joint syndrome from the same area for a year. A sport incident finished me off. I can't bend forward, as it triggers sharp pain in the area. I can squat and bend moderately backwards. It doesn't match what you say. Can you please advise me if there is anything that would fix my back for good. I a 5o an cant see myself another 30-40 years dealing with this. Thank you

    It can happen while doing dead lifts ? As you said i feel relaxed when i bend forward but it's very painful to bend backwards. It also hurts while standing straight. I don't know what to do please doc. Help me..

    I was diagnosed with suspicious par stress fracture at L5 on right side according to my lumber MRI REPORTS. After 3 months of rest it is healed completely according to my current lumber ct scan reports. Overall reports are normal including all the disc height , canal, as well as no facet joint arthopathy is not there. But the problem now is that i have right side lower back pain when i over extend my spine. (while doing any bodybuilding back muscle posing ) specially it feels like the pain is in the bone or joint. Please help sir i am very much tensed about it. I used to do bodybuilding and want to continue with it 🙏🏻

    One thing i would like to mention is that the pain was gone after 3 month of inactivity at my lower back but i did extreme yoga streching and after than i am experiencing lower back pain again on my right side while extension but also i did my lumber ct scan after this only. So no abnormalities revealed please do reply 🙏🏻

    I have had two procedures for a facet injection and steroid injection for a synovial cyst. That procedure is incredibly painful. How do I stop them from reoccurring?     Hello Doctor , I have extreme pain on my low right side in L4/L5 . When my back extends it’s very painful. My doctor tried medial branch blocker and it only worked like 40-50 percent.
    I don’t know what to do. ?
    All other injections failed 😞 😢

    I’ve been for injections a few times they give me relief to a point for a few months but I’d still get pain if on my feet too long, so my next thing is verve ablation of the facets. I think I’m affected a bit higher up than what you show maybe by one or two vertebrae. The pain radiates to my hip and into my lower back. Sometimes it feels like pressure pain.

    Some pain but most tingling into my feet. MRI showed inflammation. I was scheduled for steroids injections but the pain disappeared for 3 weeks and the nerve sensations so the consultant said no to the procedure. I got so upset and then the pain and sensation came back. When I press the area it tingles more. I m see a physiotherapist but I don’t know how the sensation will go without the steroids? It’s been 5 months.

    I can bend backwards without pain if my feet are parallel. However if I put my right foot out front and then bend backwards, the pain is intense. Is that a facet problem?

      You can't exercise a facet joint lock. It's a joint, not a muscle. That's why people are so frustrated. It's like trying to stretch diabetes away...sure stretching might be good, but it's not targeting the problem. If the facet joint doesn't get the movement restored, early degeneration of the cartilage and chronic stiffness results within a decade.     I have facet pain but I also have extruded discs from L3-S1. With the exception of the hand manipulation technique you showed, are the other recommendations ok? I have been doing them for several years for relief.

    What about adjusting the facet of the T1-T2?? Pain only on the right side of the spine. Any thoughts? No one seems to be. Able to help me and it’s been 8 years of this pain and thousands of dollars and a list of doctors who just won’t listen to me. They keep looking at different areas and not treating the area or giving me any PT that helps

    Hi Doctor, I am not able to bend forwards. When I try to my leg muscles pulls and stops me from bending forward. My body sort of bends in a tilted way when I try to bend forward. Is this because of facet arthropathy or does it have something to do with the muscles in the leg? I have sharp pain in the buttocks when I sit for a long time and when I lie on my back. Sometimes I get shooting pain down the leg too. When I get up after lying down for a while, I feel stiffness and discomfort in the lower back. Do you think this is because of facet syndrome? PS: ever pain I mentioned is on the left side.

    I recently had radiofrequency ablation done on 4 facets on each side. It was done a few days ago and I'm feeling better. It may last 6-12 months. What can be done to remedy this problem before it comes back?

    Thank you for this. I'm seeing a DC now for facet joint syndrome that I believe was caused from a back spasm as my x-rays show no other skeletal abnormalities except that the facets are out of alignment. It is very painful to arch backwards as you suggest. Is it also normal for facet syndrome to cause pain radiating to the thigh? it's a pretty continuous deep aching pain and difficult to find relief.

I’ve got a question, I’ve been suffering from facet syndrome since 2013 and I’ve visited 7 doctors so far and each one of them said something different than the one before him. I do believe that my injury is not too bad because I can do what most people can’t like running, playing soccer and working out. But these days I feel some kind of pain in my back maybe because I’m pushing myself a little bit too far. So, what exercises might help me strengthen my lower back or relieve my back pain?

    Is it possible to get facet joint syndrome around the T10 vertebra (lower thoracic spine)? I’ve had back discomfort on my right side of the spine and a couple of “knots” that appear to originate around the transverse process area and extend past the erector muscles. I’ve had this for about six months now. Back flexion is the one movement that feels restricted and don’t have very good range of motion. Thanks!

      See a chiropractor. Pain during walking usually indicates an SI joint dysfunction. Chiropractors are good at helping that

    I have been diagnosed faset joint arthritis. And reduced lumbar lordosis with left side scoliosis in the thorsal segment. My disks are thined. I have an inflamation in the thorsal segment.

    Do you have a video about multifidus stiffness? I don't know if I have a facet syndrome or multifidus, I think the second one from what I have learnt on CZcams.. Anyway, is it safe to do this release manovre with a multifidus stiffness? Thank you

    Hi there, I am a fellow cyclist.... unfortunately I was knocked off my bike by a car last year and broke my back and neck. A year on and I am experiencing lower back pain.... awaiting appointment with orthopaedic surgeon but my physio thinks it will be facet joint.
    You mentioned in your video about core exercises for facet joint issues. Could you point me in the direction of the video? Any advice specific to cycling would be much appreciated too. Thanks for the informative videos.... very helpful. James

    So i have no strength in my left thigh. I figure the femoral nerve is pinched. The femoral nerve comes out of L3. So is it safe to say that the L3 facet is the cause of my muscle failure?

      Oversimplified, but good thinking. The femoral nerve is L2-4 and there can be other causes not neurological that could cause your thigh to feel weak. See a chiropractor.

    Thank you for taking the time to help us.. You spend more time with us on you tube than my neurosurgeon does with me in person. So how do I add more range of motion or stretch since I've had a laminectomy and fusion at l3,4and 5? I also currently have a fracture in my l5 vertebrae AND l5 left pedicle screw . And bilateral facet arthropathy at l5 s1, and it's only been 3.5 yrs since my surgery. I don't even know how it got fractured. I think my Dr doesn't know where to go from here. .and neither do I . I don't kown what stretches im supposed to do or not do ..Can you help at all with this?

      @Frank Lopez Thank you 😊 I know an internet dr probly isn't gonna help me but it was worth a try lol. It's not that the treatment is bad, my Dr is just very brief. All my scans are up to date and I recently had a mylogram. At the end of the month we are going to be discussing what surgical options I have and what can help the pain and prevent worsening. It's def a long road. I don't think I'll ever not be in some form of pain or recovery. I'm sorry for your pain as well. Thanks for the kind words. I hope we can feel a little better 🙏

      @Frank Lopez Yes the Healthcare system is def crap. And yes spinal pain is the worst. It does get depressing sometimes-especially when you cant do things you uses to love to do. Its hard. It was nice meeting you too tho. I actually use this app called The Mighty, and they have a lot of supportive groups for chronic pain and different mental health issues and it really helps so much. It's a community of ppl just like us and they are very supportive and understand. You should try it out if u feel like it. It's not like other social media. It's actually positive and ppl on there all help eachother. Maybe I'll see ya there. Good luck with everything and if u ever need to talk- I'm here 😊

      @Frank Lopez well I haven't seen anyone be mean, they have a lot stricter guidelines than other social media in that sense and I'm sure u will see a little misinformation anywhere u go ya know? But I think theyre pretty good about catching it. Plus what works for one person may not for another even if it is a truth. I'm from CT. I lived in FL for a while in Clearwater. I love it there.

    Hi doc plz help ...my MRI says mild facetal arthropathy at L3-L4 and L4-L5 Levels.. What does this mean is this curable by good posture and exercises...or it's gonna worsen up in future...

    Any idea on how to help those of us who have Ehlers Danlos Syndrome? I’ve had terrible low back and leg pain for 3 years and my MRI shows facet arthrosis in L4, 5 and SI. Then it says STIR hyperintensity between the same L4 to Si that may reflect edema and bursa formation. I’ve done all traditional therapy and nothing helps. Can your spine continue to send more and more pain signals to the brain with this? Hope I’ve made sense.

      Good question and it's a treatment-dilemma because there's just not a lot of research money to figure out "what to do about it." My experience with it is that manual adjustments and full range of motion exercises don't help. Arthrostim and short range of motion stability exercises can calm the pain down.     I am a 72 year old woman who has been dealing with severe back pain thouoght to be spinal stenosis, and Finally deciding to have a laminatomy and fusion. in the interium i am seeing a pain management doctor who has now said he thinks it is my facts joints full of arthritis. I am overweight and presently trying to lose those extra pounds but my question is, should I disregard the diagnosis of spinal stenosis, abandoning the surgery idea, and try healing the facet joint pain? I recently had facet abalation on the right side and I am scheduled to have the left done in two weeks. The pain on my right appears to have diminished with this procedure, but the left has gotten more severe. At this point I am totally confusied as to what I should do.

    Using your method for correcting facet syndrome, how long does it take for a patient with a severe case of it to recover? I've had it since last Sunday, and as long as I have this pain, I can't work. My chiropractor says it's going to take time to heal, that it would be a miracle for me to be able to go back to work in 3 days, and that it could take as long as 3 weeks. He, his wife/receptionist, and a patient in his waiting room have all suffered with facet syndrome, and all have recovered, and are confident that I'll recover too. The more treatments, the quicker the recovery they say. (OUCH!! I just sneezed! Oh, that hurts. I put some ice on the painful area.)
    My doc also recommends an exercise where you lie on the floor and put your legs up on a chair, bed, or box at a right angle, breathe into your abdomen, and make sure both hips are on the floor. I did that for an hour last Monday. It felt o.k. while I was doing it, but afterwards my pain increased a little bit. He also insists that I apply CBD cream to the painful areas every 3 to 4 hours to bring down the inflammation.
    What are your thoughts?

    I find bending forward is more stiff and tender . Catcow stretch is where I really feel the soreness right in the lower lumbar joints. Any advice ? I go for physio . I’ve had injections and a nerve ablation and will be going for a second nerve ablation but I’m tired of being in pain and seeking better relief at least until I visit the pain clinic again.

    Hi, thanks for this. I think i have facet syndrome but I have a history of rear bulge at L5/S1. I have seen a McGill provider and I have made progress in eliminating the disk pain. However I exaggerated extension and I believe that caused by facet irritation. I've reworked my body mechanics to keep my spine in neutral but I can't get the facet joint pain to go away. I have even iced three times a day and done decompression stretches 10 times a day and nothing seems to work. The McGill doctor is uneasy about doing Chiropractic adjustments and I understand but I am looking for a good chiropractor. Do you know any chiropractors that live in the upstate New York or New York City area?

    You said you do this adjustment on patients as long as they don't have a bulging disc. Is there any adjustment option for patients that have a disc bulge and degenerative disc disease with the facet arthritis? My husband has this, we go to a chiropractor regularly but I didn't know about adjusting the facets like this.

He has had lumbar nerve ablation once so far and the pain clinic determined the majority of his pain is from the facet arthritis. He's only in his 30s so neuroscience says he isn't a surgical case.     Proceed with caution when squatting or twisting as this can also cause severe pain for those with facet joint syndrome.

      Same at L4-L5, Fusion did not change anything. Slightly more pain all 3 yrs after surgery.

    Thanks yeah I have started Zumba and feel a lot better, staying low impact but also need to lose weight, now not having pain as often down the leg ... I do have stenosis. Neck, middle, lumbar I suspect, have t had MRI of middle but it appears in the other regions... I am 58 but I joke that my spine is 68... weight loss will really buy me some good years I guess. I need to lose 30 Lbs.

      You'll need an expert chiropractor. They can open up the joints, allow greater joint play and possibly use cold laser therapy to reduce the inflammation. This is how I do it: czcams.com/video/My3Hx2QC_EU/video.html

    You talked about hypermobile facets.. What if a patient has a hypomobile facet. Would the treatment be the same? How do you diagnose hyper vs hypomobile facet syndrome?

      Diagnosis is via motion palpation. One could use motion x-rays, however, that's a lot of radiation only to discover what a good pair of hands can discover (for personal injury or workers' compensation it's often necessary to do x-rays to justify treatment). Hypomobile facets get adjusted.

    Hi Dr. Story, thank you for posting these videos. I had a CT done of my lower back area and found that I had slight protrusion of the L4-L5 and L5-S1 discs. But the doctor also diagnosed me with facet syndrome (which, by the exercises I've done and the explanation of symptoms, seem to be the correct diagnosis). It seems like what's good for facet syndrome is bad for disc protrusion. How do I reconcile with these two different needs? Are they at odds?

      Great question. The best way to deal with those types of patients is to treat the patient based upon how they FUNCTION rather than what you SEE on a CT scan. Sounds silly, but we don't treat the CT scan, we treat the patient and attempt to get them to feel better not by masking the symptoms, but by trying to restore proper function and motion in their spine. So if the signs point us to treat the disc even though the CT says facet syndrome, I would treat the patient as if they had a disc problem (see other videos). I always look at a patient and ask myself, "what can they do and what can they not do?" and "how is their spine functioning (motion in all directions) and where is it not functioning well and how do I correct for that?" In your case, one is likely dominant and I would treat that one in my office.

      As a follow up, what started the acute instance of pain was me bending over to reach something. After a few days that has subsided, however, I only feel more pain when I bend backwards. Isn't it odd that stretches aimed at relieving facet joint syndrome encourage bending forward yet that's what caused the original spout of acute pain?

    Is facet hypertrophy the same as facet syndrome?
    I have a disc protrusion and facet hypertrophy in lower back.What’s a good exercise for these two conditions?

      facet hypertrophy means the facet grew bigger due to the stress on it. In theory, one could have hypertrophy that is noticeable on an x-ray/MRI/CT scan but have no symptoms. Facet syndrome means your are getting pain from the facet regardless of it's size. Go to the Playlists section of my channel and watch all 19 videos on the core and get to work.
