Assessment of Hemodynamics in Critically Ill Patients

  • Effective hemodynamic management is crucial in critically ill patients, and involves early fluid therapy and administration of vasoactive drugs to maintain vital organ perfusion and oxygen delivery. Proper understanding of the different approaches to hemodynamic management during resuscitation and further management is essential in order to initiate timely interventions. Achieving a balance between organ oxygen delivery and consumption is paramount, with cardiac output being a key determinant of oxygen supply. However, it is equally important to assess indicators of oxygen requirements to ensure adequate supply. Various monitoring systems, both invasive and non-invasive, have been developed for hemodynamic monitoring in critically ill patients, but require validation. Accurate data acquisition and interpretation are essential for making suitable management decisions. This mini-review provides an overview of the available techniques in hemodynamic monitoring for critically ill patients and summarizes their advantages and limitations

    If patient is hypotensive systolic Bp 2 mmol/L
    Do ScVO2 and CO2 gap
    If
    ScVO2 less 70%
    And
    CO2 gap less than 6 mmhg
    Check Spo2 and HB
    If CO2 6 mmhg or more
    Its associated with low cardiac output state I. e sepsis induced cardiomyopathy
    If ScVO2 is more than 70%
    and CO2 gap less than 6 mmhg
    Its high output shock state like Septic shock
    If CO2 gap is 6 mmhg or more
    Its late sepsis with microcirculatory failure
