In 2019 my wife has filed 420 and 498A, now in Aug 2022 she filed 12A section for the nullity of marriage. How can I defend myself in this case. I already have medical reports in this case

The relationship comes under Sapinda and therefore it will be void under Hindu law.

The boy and girl don't fall under Sapinda as far as new Hindu Law is concerned and therefore they can marry. However, it couldn't be done under the old Hindu law.
But marriage to second cousin isn't considered healthy due to medical reasons attached to it.
