It depends on the game. It's not common anymore, as far as I know... but PC games tend to have "developer console cheats" if you know how to unlock the console. Spyro Reignited and Crash 4 both have cheats last I checked, but they are reboots of old games.
Yes. For example: Beating the game once unlocks Symbiote Spiderman Costume. Beating it a second time from a continue will also unlock a special costume. The higher the difficulty, the more it unlocks to beat the game. I'm not sure that every costume has a legit way to earn it, though.
