    I had a double whammy congenital bicuspid aortic valve and severely calcified I had a valve replacement 8 weeks ago at RPA Sydney my surgeon is Dr Plunkett and I'm 50 years of age great video

    Sir I have Bicuspid aortic valve.iam 53 IAM taking Nebicard 5mg I can walk 4kilometer every day ..I get tired once in 3 days. Any treatment I have to take ? If you can give some opinion I can follow sir . Thank you God bless you.

    Just as a tidbit - people get confused if they have a question with a patient who could have a bicuspid aortic valve with stenosis vs. HOCM - they both are a systolic crescendo-decrescendo murmur. There are three differences between the two that help you out (if the question doesn't tell you about a bicuspid valve or a family history of people dying of a heart attack in their 20's or 30's)
    1. Aortic stenosis has an opening snap in systole. HOCM doesn't.
    2. AS gets softer with handgrip (increasing afterload). HOCM gets louder. Why? AS has a huge pressure difference between the left ventricle and the aorta. If you can "equalize" the pressure between the two, the murmur goes away. HOCM is the opposite - an increase in afterload or decrease in preload will force the mitral valve to "stick" to the IV septum
    3. AS, as mentioned in the video, can have diminished and delayed pulses ("pulsus parvus et tardus.") HOCM will have BRISK upstrokes that are quick.
    Hope this helps someone out. Murmurs can be confusing
