- Turbo Lines flight 346 was a scheduled cargo flight en route from Terminus-Jarvuni Airport to Beach National Airport on 16 April 1993. The plane was supposed to took off from runway 02 because of an airplane crash few days ago. However the plane taxied to the wrong runway did not aware of an airplane debris. The plane hit the front section of the crashed plane fuselage, Stalled and crashed. 3 of the 8 people onboard survived the crash.

Terminus Field Airport disaster
Turbo Lines Flight 346 (callsign: TOL 346) was a domestic cargo flight en route from Terminus Field Airport to Terminus Main Airport. On January 1st, 2007, the Airbus A400 crashed shortly after take-off following a collision with the nose section of XV-40, which crashed on December 28, 2006, and was supposed to be delivered to Turbo Lines. The A400 stalled and crashed onto the runway, injuring both pilots aboard and damaging cargo.
*Planes*
The Airbus A400 operating Flight 346 was delivered to Turbo Lines in 1998 (registration 9M-MRO) and had logged 14,000 flight hours prior to the crash. It is powered by four Europrop TP400-D6 engines and had no other incidents, except for a damaged landing gear during a rough landing.
The XV-40 was just 2 months old and was flying to Field Airport for delivery to Turbo Lines. It was supposed to be the first XV-40 for Turbo Lines. Following the crash, Turbo Lines canceled its XV-40 orders and started ordering C-130s.
*Crash sequence*
The XV-40 took off from Terminus Mountain Airport at 10:12 am (EST) and was due to land on Field Airport. Upon landing, the pilots noticed the brakes failed to work, and did a go-around. However, when the pilots turned the plane back toward the airport, the pilots banked the plane dangerously left. Control of the plane was subsequently lost, and the aircraft crashed on a flat area between the airport and the hills next to the airport. Burning wreckage came to a rest on the airports’ only runway, which blocked take-off from runway 33. Emergency equipment came to the wreckage a minute later, but the 2 pilots had already died due to smoke inhalation and fire.
A few days later, Turbo Lines Flight 346 (which was parked when the XV-40 crashed), was set to take-off with 10 tons of cargo. It was supposed to taxi to runway 15, but taxied to runway 33 instead. Despite not having clearance to take-off, the pilots attempted take-off, and at 2:34 pm, the plane collided with the partially burst nose of the XV-40. Engine #2 and the undercarriage, along with the section that the undercarriage was attached to, were torn off. The A400 quickly stalled and came crashing back down onto the runway. Although the aircraft broke up into 3 pieces, it did not catch fire. The captain suffered a fractured right shoulder and a dislocated left knee, and the co-pilot suffered arm injuries.
*Aftermath*
Terminus Field Airport was closed as a whole, and stayed that way until a week later on 12:13 pm where the wreckage of both airplanes were cleaned off. Safety features were installed at Field Airport to prevent another crash like this.
*Investigation*
An investigation showed that the XV-40 crashed because of a fault with Engine #2, which was linked back to when the plane was built. The brakes failed due a hydraulic leak which severed control to braking. An extreme bank angle, caused by pilot error, also contributed to the crash
The A400 crashed because the pilot taxied onto the wrong runway, and took off without clearance. The cause of these errors are unknown, as neither pilot said why.
Notes:
Identity of the pilots on both planes are unknown. What Flight 346 was carrying in its cargo is also unknown.
