It was Airbag / handbrake failure I know 1 of the recovery drivers that recovered the lorry, moving the train somehow damaged 1 of the Airbags and when the driver went to get his paperwork signed he looked out the window and just saw his truck take off and the injured person was the truck driver running after it trying to stop it luckily he hit the side of the truck and didn't land underneath it. The story was also in the paper.

Rather ironic that it is carrying a London Underground battery loco on the trailer. The loco is fitted with wheels to run on rails, so, assuming that it was disabled, if it had to be somewhere else, it could have been towed. If an axle had seized, a "skate" would have permitted movement. I admit that I do not know all the facts here, but too often rail vehicles are moved by road for cheapness- well it hasn't been so cheap in this instance.
Having said that, however, I must point out that Allelys transport are a well respected firm with plenty of experience in this sort of move. This incident surprises me as I have seen quite a few videos of similar work by them and they have shown tremendous skill.

Air pressure in truck brakes works two ways; it applies the service brakes and releases the parking brakes but there's a limit to the spring force in a parking brake even in an STGO 2 category vehicle and the video plainly shows both the STGO placard and the abnormal load.
My guess is whatever the true cause of the failure, they'll blame the driver for not additionally using wheel chocks.

Just so people understand, the air in a truck brake system holds the brakes off, releasing that air will apply the brakes, its fail safe, if you lose the air from the tanks or a brake line breaks the brakes will automatically be applied. You can have brakes fail if you use your brakes heavily and constantly such as going down a steep incline ,the brakes will overheat and you will get brake fade.

Option one is he forgot the handbrake.
Option two is he forgot the handbrake but the air was low so as soon as it built back up, it released the brakes.
Option three is the same as option two except he mistook a brake warning light for the 'handbrake on' light.
Option four is that the entire braking system failed.
Me personally, the first thing I do instinctively is engage the handbrake. I usually double check before I leave the cab.
