If you can share on how to write test cases rather than just the structure it would be very helpful . Also how to test not the typical gmail or website login but something where you share the creative/logical ways to test. We know positive and negative what else can we think in order to do testing.

The test case are written based on requirement , reviewed and approved .now test engineer finds many more scenarios while testing the application . So the new test cases he will write again , get it reviewed and approved ?

We shout wrote it in test case tool, like jira, test rail, test manager etc.
Startup companies even write it on excel sheet.
