This was awesome! There's a few things I wanna point out to help you understand what happened:
You were suddenly at 2 health right before the Quillback bossfight because you got hit with your own curse projectile, then leveled up and immediately put a level into health, which gave you an ailment cure plus one extra health.
The "hunter" you kept coming across is part of a karma mechanic that works under the hood in the mod to punish you for murdering shopkeeepers. Since the regular shopkeepers aren't present in this mod, there needed to be a character who gave you an incentive to not murder and rob from random people, this became the "bounty hunter".
Basically, everytime you steal or piss off a shopkeeper or NPC, it gives you points of "karma". I forget exactly when they cap out, but the higher your Karma is, the more likely the bounty hunter is to spawn. The only way to get karma to diminish is to kill the bounty hunter herself.
The problem is that you were constantly murdering every single NPC, so your karma levels were probably to a point where you would never get her off your back.
With that being said, I can't thank you enough for playing and enjoying the mod. It still frankly feels surreal. Maybe someday there will be a full release.
Also, I wouldn't mind seeing some Elden Ring videos :)

52:18 I think Nuclear Throne does it right, that's why it's one of my favorite roguelikes of all time. The bosses are pretty simple, they tend to have like two or three attacks at most and won't take more than a minute to kill (except the final ones) but they can easily kill you with just one fuck up on your part, which perfectly matches the rest of the pacing of the game rather than making it come to a halt. They are also part of the levels instead of isolated, which helps to always change it up a bit, because the map layout and enemies that surround them wont be the same every time.

this audio desync is driving me crazy
It's really odd, I changed absolutely nothing and didn't even edit this video in any way. Just a weird technical issue during recording, no idea how it happened.
Upon further investigation, everything I recorded that day is delayed exactly 1 second and I couldn't tell ya why. I did a test just now and it recorded normally. Just windows / OBS / Realtek onboard soundcard borking out in some way.
At least I can edit the synching easily for the other spelunky vidya coming up =D
