The first is said by the priest during the prayers at the foot of the altar following the praying of psalm 42, the second is prayed right after by the altar server(s) and/or deacon and subdeacon (depending on whether it's a low mass, high mass, or solemn high mass), and the third one is the one chanted that you see above, right after the Precious Blood is consumed. People often say that there are two confiteors instead of three because they usually lump the first two together, since the second one almost immediately follows the first.
At my SSPX mission (Low Mass), 3 Confiteors are said: one by the priest, followed by server’s, and the server again after the priest consumes Our Lord’s Precious Blood.

IF ONLY PRIESTS KNEW WHAT THE SECOND CONFITEOR DOES TO SOULS JUST BEFORE COMMUNION. THEY WOULD SAY IT AT EVERY MASS...
