      No. That is what physicians are supposed to do. What we should be doing, and doing more often, is taking a moment to criticize those physicians seem unable or unwilling to tackle urogenital care without awkwardness, or embarrassment. They are out there, and they are a pain in the tuchas.

      @@soilmanted Your response ties nicely into my initial question of bedside manner being taught and adopted, or being innate and developed.
      You were advised to READ "How to Make Friends and Influence People." Was the expectation that you would adopt the entire framework that Carnegie presented ("taught" your "bedside manner?") Or, would you look at the information and adopt it to what you already knew about the subject innately (innate and developed "bedside manner?")
      Thankfully, you have done the latter. You analyzed the incoming information against what you already knew, or experienced. Some people do not do this, or do it in different ways. Carl Jung mused, "“Until you make the unconscious conscious, it will direct your life and you will call it fate.” We all see the world through fundamentally different lenses and make different judgements based on what we "see" using differing criteria.
      For Carnegie, that was his "truth." It made sense to him, his cognition, and how he lived his life. Not to you. Not to others. Take a look into Jungian Analytical Psychology. I would not be surprised that you are an Introverted Thinking (Ti) lead based off of your posting and analysis. It may provide answers for you. Or it may not.

    Way back when I was 18, I got my first serious job. I was a kid terrified of displeasing the new boss, about ten years older than I. A few days in, we stood at two urinals together. He said with English accent, "no matter how hard you shake it, the last drop always goes into the pants." Truly an icebreaker!

    I turn 60 this year and have experienced this often. One simple thing that has really helped. Just stay in the urinal longer. Even when you think you are finished wait for the straggler. They will come!!!

    Just give it the "shake and fake" shake it when you're done, put it away, then immediately whoop it out and shake it again. It's a master strategy that works well for me haha

      @@EatAPeach72 try the "shake delay fake" a similar tactic I devised in which you hesitate for a brief period of 1-3 seconds before the fake out. Sometimes the dribble is fashionably late

      It’s even more deeper than that I dreamt last night about peeing which normally I wake up to wet underwear but didn’t which is crazy that I didn’t only to have wake up wanting to continue sleeping but realized just go pee already so I did. I thought wow I didn’t make a mess and it was a miracle what just happened I’m 50 by the way only to have this pop up hours later on the algorithm which blows my mind more

      This is the reason why I never go to urinals and opt to use teh toilet. I have to use a toilet paper to release the capillaric tension for the final drops...

    I used to have this problem. Changed my diet for other reasons. Don't drink soda, orange juice or any juice (it has lots of sugar). Reduced my sugar down to like one small sugary snack a day. (Take in less than 50g sugar a day). No fried foods, no donuts, no candy, etc. Lost 30 lbs in a month or so. No more dribble. No more chapped lips or split skin on fingers from the cold. Less skin problems.

      The chapped lips/split skin is generally from not enough water. Many people reduce water intake in winter due to the cold.

    i started sitting to pee at night around the late 80s ( i am 77 now). I would sit facing the tank, resting my arms and head on the tank lid to relax while i pee. This is long before troubles with dribbling. Got use to sitting and would always sit at home. The last ten years or more I'vc had the dribbling. Don't have any prostate enlargement. I have tried the massage way to finish the emptying It's difficult however I working on it. I hate this problem and I wear black shorts and underwear when possible to cover up and missed dribble. Anyhow absent any men's dribble issues I prefer to sit when confident the bowl and flow is clean. Sometimes i look at my phone to relax to help voiding. That's my story and i'm sticking to it.     For me personally, I found out I just needed to stand at the toilet for just a little bit longer. I grew up always in a rush. Go to the toilet, urinate. Squeeze out what you can, shake and think I was done. One day, a few years ago I finished urinating and did all the things I mentioned earlier only this time I just stood there and relaxed my muscles for maybe 8 seconds. Suddenly I felt urine start to flow again. It was a small amount but enough to obviously make a mess in your underwear lol. Ever since I learned this I do it all the time. Let me know if you have gone through this and if you try this method out let me know how if it works for you.

    Dr. Malik, i would like to thank you for giving a solution to my post void dribble ( which I had even when I was younger, and believe me there are lots of young guys who suffer from this condition). I used to think it was a sphincter problem at first, but when i came across this video, i tried the fingertip method behind the scrotum and I have stopped having post void dribble. I am now 52.     I have had this issue since I was kid and I'm 52 now. None of the doctors have been able to tell my why this has been going on. Nor have they given me any suggestions on how to deal with it because I showed non of the symptoms you mentioned. I have also had a very steady stream my entire life. People have always told me that I, "Piss like a race horse." However, I have always been a bit worried about this. But, other than being a little embarrassing at times. There hasn't been any adverse affects to my overall health. Thank you Dr. Malik for explaining another technique to evacuate the urine that I have left.

    When I was 26 years old I saw a Urologist for this problem. He gave me a sulfur-based medication, by the next day my life changed forever with this medication affecting the bacteria in my small intestine and resulting in Crohn's Disease. That was 23 years ago. CZcams videos and the creators are a great gift to us, thank you.

    I think a point that is being missed here in the video is that the urethra is a passive tube similar to a water hose and does not contract like a muscle to expel every drop of urine. If you cut off the source, the bladder or faucet, there is still water or urine in the pipe. In the case of the urethra, it is even worse because when you urinate, there is a natural curve going up the scrotum and down the penis. This is a natural trap for urine to get stuck to work itself out later with movement and especially sitting down in a chair as this presses against the urethra behind the scrotum. Because of this natural curve or trap in the urethra a small drop or two of urine coming out is normal for all ages and can be remedied by pressing the urethra behind your scrotum wirh your hands after urinating. If you have other symptoms on top the dribble or excessive dribble, its best to see a urologist especially if you are a older male. I am not a doctor and had a drop or two of urine come out that i first noticed in my early 20s and this is the information i found with my own research and going to see a urologist in person.

    Thank you for the technique here. I didn't think of it before and it really helps. The dribble was extremely annoying. The only issue now is when going through clothes and access is a little trickier, but I found applying pressure through the clothes to the area also works sometimes.
    I do agree with what others have said, I appreciate that you address the "do not talk about" topics that society catagorizes them as, yet they are very important and health progressing.

    I have had increasing issues with dribble and wet underwear since about 24 years of age. I had all the symptoms you mentioned including stopping and starting and night emesis. Now i know why nothing seems to help it lol. Im 51. I need a urologist. Thank You 🙂

    I had this issue in my early teens, and I believe it was merely due to the growing muscles, plus I have a seizure disorder. Once I turned 18, I no longer had the problem and I am now in my mid-50s.

    For me I noticed it is heavily tied with stress, particularly relational stress. When I am stressed out i notice that my pelvic floor muscles tend to be stiff and dont respond well to stimulations from my brain to release urine. However with Yoga and pelvic floor stretching and relaxation, I have noticed a MASSIVE improvement. I used to get to that last little bit of urine in my bladder and i could feel it just not wanting to come out no matter how much i relaxed and some of it would come out when I sat down. Now it feels SOOO NICE for the little bit to come out and feel satisfied my bladder is completely empty. When I do that final little pinch the last little bit squirts out. Interestingly, I never found out about pelvic floor issues being tied to emotional stress until i did my own research. Even my urologist was like I have no idea what could be causing this.

      I think there's a connection between all sorts of prostate issues and stress. I read the book Teach Us to Sit Still and it was really eye opening.

      From what I understand its actually not a prostate issue until it is a prostate issue. In other words, the muscles down there that aid in urination and ejaculation all become extremely tight or weak and tight. They are directly connected to our diaphragm and the stress on our gut ties into it.

    Thank you so much. I have noticed that energy drinks exacerbate this issue, and sudden intense urges to urinate, which you have addressed separately. Your genuine empathy, while giving what I consider critical information, is wonderful. You bring what I need to hear alive, so-to-speak. As a man in my 60s, I am in a club where we are all undeniably older now. We see the world we left, every day, always. It is hard sometimes.       i was drinking 3 - 4 energy drinks a day, i have been thinking that it could cause the issue, however no data shows me that energy drinks cause it. i just quit energy drinks 5 days ago.

    There can be a few reasons for this. But for the average person, once your muscle to your bladder closes, you still have urine in your urethra. In order to get most of this out, you don't just shake. Although you may get most of what is near the distal end of your urethra out, you are still missing what is more proximal. What you can do is give a little pressure upwards under your perineum. This will help move urine that is still stuck close to the bladder muscle further down the urethra. Then you start at the shaft and gentle 'milk' the urine out. Then give it one final shake.
    Other reasons maybe a very tight sphincter muscle(s) that don't allow the bladder to drain. You may also have an enlarged prostate that sits right under the bladder. As we age, ALL men's prostates enlarge. There are medications you can take to help with this. The most common one used is called 'Flomax.' Also, it is more difficult to empty the bladder after sex. When a male/man gets an erection, it helps prevent semen from entering the bladder. It basically holds the muscle of the bladder shut. Other men may have over active bladders. There are treatments for this with Botox being one of the more common. Using certain medication like narcotics can prevent the muscles from relaxing as well. You should avoid anal sex as it is not natural and you can not only get a severe UTI, STD's, but also cause severe anus and rectal issues. We have seen a lot of people who engage in this behavior in the OR. The rectum is a waste management system. Intended to get rid of waste. I cannot tell you how many tears we have fixed and resections from people sticking things up there that don't belong. In addition, you can cause potential cancer cells that might be in the bladder and/or prostate to spread by putting pressure on it from sticking things in the rectum. You can also damage your sphincter muscles as well.

    The main cause of post void dribbling is the retention of urine in the urethra. The urethra is a passive tube, it does not have muscles to propel the urine out. It is analogous to a garden hose, you can close the tap (sphincter) but when you pick up the hose the water drips out. It is almost never related to a stricture and any urine in the prostatic urethra would not be cleared by milking of the urethra or Kegel exercises. It is not exclusive to old men, and happens to younger men and even women and children occasionally. And yes I am a urologist.

      I have felt that in some cases there is a neurological component. Perhaps the autonomic system can influence the neurology in such a way as to affect voiding.
      Consider that essential hypertension can be caused by neurologic bias at ganglia proximal to the kidneys. Often times stress hormones exacerbate this condition. Maybe that same autonomic process - related to stress hormones - can effect voiding in the same way?

    Great video Doctor Malik. Can't speak for others, but for me it seems particularly prevalent when I'm super bloated - IBS like feeling, or ascities symptoms. Basically, it inhibits my ability to push the residual urine out... Just super frustrating having to park at a urinal (especially if it's public), and swing it out for a few minutes. Who wants that sort of weird attention.

    I had the condition in my early 30's. It turned out to be severe candida albicans overgrowth and cleared up with taking powdered nystatin mixed in water 4x daily for a month, in increasing (small doses). Likely caused by long-term use of Tetracycline (antibiotic) to treat acne which was commonly prescribed to teenagers in the mid 60's to mid-70's. It killed off all the good intestinal flora, let candida grow unabated and had to be eradicated in the gut and urinary tract with an antifungal treatment (nystatin). I also took a good course of probiotics. I'm 66 now, and there has been no recurrence.

The complete workup by Dr. Wright included urine, stool and blood samples where they either found and cultured candida directly or found candida's markers. There may also have been some scrapings of the tongue where candida presents as thrush, or of the sinuses. There was another factor with me, concerning insufficient stomach acid, called hypochlorhydria for which Dr. Wright prescribed HCl with pepsin capsules taken with meals to better pre-digest food in the stomach. People with low HCl have something called intrinsic factor that impairs the ability to metabolize B12. Apparently an acid poor gut contributes to candida overgrowth.
      In the years since, I have not used Nystatin again. Though I have had candida flareups as often as I take broad spectrum antibiotics for an infection. The moment I've finished the antibiotics, I purchase a good anti-fungal and a probiotic acidophilis to take for a month. Cranberry juice helps too. For an anti-fungal, I've had good success with Enzymedica Candidase. I'm not choosy about the probiotic acidophilis. Presently I'm using one I get from Costco (like Sam's Club) by TruNature and I take one every morning which has virtually eliminated my GERD. God bless!

      @@Unlimited0X My original reply was removed; I believe I understand why. The Nystatin treatment I was prescribed was probably off book and not safe. Turns out Nystatin comes in a powdered form for external use, and in pill or liquid form for internal use. My memory above is not faulty in the matter; I was instructed to mix nystatin powder in water and drink it 4x daily in increasing dosages for a month. But that was in 1986. Either it was a food grade / oral type of Nystatin I can no longer find, or the doctor had me take an external one internally while supervising me closely. Suffice to say it was not a pleasant experience between the nasty taste and the candida die-off. I looked at the doctor's web site and they no longer list nystatin on their treatment or pharmacy pages. Instead they sell OTC antifungal capsules. There are forms of Nystatin intended for oral/internal use in pill and liquid form. They require a prescription. Reviews say the pills are the nastiest tasting thing on God's earth as the pills are to be held in the mouth as long as it can be tolerated before swallowing. Search for "nystatin oral". I use Enzymedica Candidase for occasional candida flare ups. It seems to work as well as Nystatin did for me. God bless!

    I tried the push from behind the scrotum manoeuvre and I was amazed how well it works every time and also how much is expelled which it cannot be that healthy to leave there. Completely did the trick for me. Many thanks.

    I’ve struggled with this issue and other RELATED issues, up to and including bed wetting, well into puberty. I’m sure this condition has affected my life in a few ways, primarily causing actual emotional and social difficulties throughout my entire life. I have selected jobs solely on the ability to have access to a bathroom so that embarrassment is hopefully reduced to manageable situations.     I’m 81 and everything you mentioned here I’ve got. But the “real joy” comes with the use of a catheter. I’m glad I found your channel. Maybe you have, or will, talk about catheters. That seems to be the end point after the pills, the stinging nettle roots and the laser surgery have had their day. “Sorry sir, there’s nothing else we can do, unless of course, you want us to take out your prostate. But that has its own problems.” I, myself, haven’t quite reached this point yet, but each day is an interesting struggle and the box of catheters sits in my bedroom waiting….waiting. But that’s better than a desperate 911 call. I’m trying to be philosophical about this BPH thing. It’s better than cancer any day.

      Hey this only happens to me when I am at public bathroom. I feel like some one is waiting there or watching me, I stop the urin quick, and when is time to zip up more urin continuous to drip. It not a condition, just pure shyness. The fear of some one rushes me to leave the bathroom early than I want to. You can nick name it premature finish urinating.

    Yep, I turned 50 y.o. this year, and this happens about once a week. I wasn't aware this was a thing, thought I was just rushing to get finished. Upon putting it way, suddenly one last squirt hits my pant leg; for crap-sake! But now I know and can deal with it better. Thanks, Dr. Malik! 👍

    59.6 and began at 32 years old, urologist never explained it as clearly as you and as a mechanical engineer, I appreciate it. It has worsened since developing hemorrhoids, hot seat on motorcycle apparently partially caused them, anti-inflammatory gel helps a lot and noticed reduced dribble when no flare up.

    I always thought that was just me! I usually go out and walk my dog but i always try to avoid going for a wee before as i dont want to leak through my trousers before i go outside.     I've had a bit of this problem for decades, since an inguinal hernia operation, which seems to have somewhat damaged the outer prostate valve or its nerve. But pressing on the prostate and a good shake is usually enough to keep it from being a real problem.

    I cant express how helpful this presentation was for me.I have been treating with a local urologist for about four years but haven’t had improvement with reducing my 5-6 times night time peeing routine. In addition, I do occasionally have the dribble and splayed stream issues. I’m only 71 years old. You have been great explaining in easily understandable terms. Thanks a lot!

    Dr. Malik, thank you for informative content. I just had the HoLEB procedure a month ago which has solved many of my problems. The one piece of advice you gave in this video that I can't stress enough is if you have urinary tract problems get a Urologist. If I had it to do over I wish I had seen the Urologist years ago.

    Haven't watched the video yet but I find that sitting helps, especially if you lean forward at the end of the stream. I got used to sitting as I lived in a an all female household and I find the above described method works.

Im not sure how often you clean your toilet a week, but if you stand pissing in a bowl you should at least clean it daily ... or have a urinal installed ... But standing in shared urinal is just gross for whoever comes after you ...

    Pelvic floor tension may be a reason for exercises to relax those muscles, but this tension of course has a cause. That should be addressed too imo. Sitting behind a desk in one and the same (tense) position (office work, gaming), supporting all your weight on the same spot in the lower back (where your bladder nerves are) may not be very helpful for a good pelvic floor and bladder condition. So prevention may deserve some attention too. Changing these these habits may save you visits to your medical doctor. More in general posture is a thing in this.

    Thank you Doctor Malik for covering a subject I never thought I'd never see. In my case, every time I used a urinal, I had the dribble as soon as I put it back in and it relaxed. It didn't matter how long I stood there and shook it. Could it be the angle of attack when peeing? Could it be closing off the urinary tract as you lift it to pee in a urinal? No matter how long you try and shake it, it is always being closed off. If I had to pee in a bottle and it was relaxed, I didn't have the dribble after. Thanks again for bringing up the subject.

    Dr. Malik ive learned more about my prostatic health in these few months of watching ur videos than the 20 years of visits to my uro here in Chicago, thank you. Im 65 and ive been having issues with my prostate since i was 32! Ive had prostate cancer for 4 years now. And im about to have 2nd green lite therapy procedure. Not looking forward to that one again, but its necassary. Hopefully this one will last a little longer than 3yrs like the previous one did. For you guys who are on alfazosin, if you discontinue use, ur ejaculate will return. Anyway, thanx, and stay safe!

      sorry to hear that i have prostrate caner since 2017 i was told it was very small have to have a biopsy every 12 months and psa bloods every six months i live in ireland europe im 57 yers old

      @@corkion Hi Con. You must have very low grade prostate cancer if they're happy to monitor you. Rest assured you're in good hands once you're in the system. I live in Dublin and finished treatment for prostate cancer in 2017. I was caught early and thankfully I'm doing great. I have PSA blood tests every 6 months. I cannot speak highly enough of the medical staff and the way they've looked after me. My best wishes to you.

    Thank you for this video. I am turning 60 this year and I do not have any of the other symptoms mentioned in the video. I only have the issue of dribble after I thought I was done peeing. I'm going to try the massage technique recommended here and see if that takes care of the issue.

    When I was in my late thirties and in my forties, I was told my prostate is quite small but I might have problems with retention - that was the term. I was taught the exercises you mention, and I still do them, but a bit differently. I was taught to squeeze and relax about 10 times in succession - short duration. That has worked great for me, especially since I drink lots of water day and night. I am now 79.

    Turning 70 in July...so I'm getting older. Also, I had external low radiation and internal (Brachy) high radiation treatments for Prostate cancer one year ago.     I had this problem and thanks to your video I discovered a way to avoid dribble. After I'm done I relax my prostate and let the rest of the urine slowly drain first. I also take Tamsulosin 0.4 mg at bedtime. The only side affect is dizziness because it lowers my blood pressure.

    I've had trouble urinating and felt like I was sitting on a golf ball for about 7 years. Read somewhere that pumpkin seeds might help. I started buying pepitas (which are just shelled pumpkin seeds) and after about 2 weeks it was night and day difference for me. No more pain from urinating, testicles didn't feel like they were being crushed in a vice, I don't lay down to go to sleep and have to immediately get back up to release a thimble worth of urine. I eat a handful of pepitas each day and haven't had any problems for about a year now. Just something that might be worth trying if you're suffering.

    I started taking a collagen peptide powder (20 grams per day, dissolved in water) and after about 90 days, my dribbling problems improved by 95% and have not come back. This has been such an improvement in the quality of my life that I cannot put it into words. I wish some serious scientific research could be done on the effects of collagen on the bladder and urethra.

    Was in a 5 star hotel in China - there was what looked like a hairdryer installed on the wall in the men's room - I mentioned this to my Chinese associate - he put me right - it was a pants dier for this dribble problem. So civilised the Chinese.

    I also generally find a gravity assist helps. Hydration and going when you have a full bladder rather than at the tiniest urge to pee, and generally relaxing and taking your time when you do helps quite a bit. Obviously don't hold your urine for long periods of time, I'm talking about going and drinking some water and waiting ten minutes instead of going to the toilet straight away.
    Also Dr Malik is 100% right, having strength in your pelvic floor muscles helps a great deal. I have control over that valve in the bladder and can start or stop a stream at will. Don't be afraid to treat your pelvic muscles like any other muscle group, take care of them. Be familiar with your body. Massage the area if it's tense or sore and do exercises to improve control of those muscles, they are yours after all no sense in opting not to use them if you can.

    So far, the only symptom I've been experiencing is hesitancy, but I've had that problem for quite a while. For me, it's simply a matter of relaxing the involved muscles to prevent dribble afterward.

      When I was about 15, my first job, I was having a leak at the urinal next to the personnel manager. He said those exact words to me. Never forgot it, still say it. Even though at 15, never had that problem then. I’m 75 now, and it rings true.

    I started sitting down on a toilet to urinate in my '30s. It is more relaxed, and easier to completely empty out all the plumbing than standing up. This was suggested by a woman who knew a lot about men. Public urinals can be very stressful.

    I have scar tissue in my utetha from a STD I had decades ago. Been dealing with dribble since. Also, I'm in my mid 50s so yeah... Prostate seems to be getting larger.
