I live in Arkansas. I would be considered middle class according to my income and I pay several hundred dollars per month for my health insurance. At the same time some of my wife's family whom would be considered "Poor" pay nothing, zero. I'm not complaining I prefer to pay my own way but what you're saying isn't true from what I see. Also one's health to a large degree is controlled by that person's decisions in life.
