As a paramedic, I had a call 40 years ago for a man injured by a welding tank explosion, probably caused by a flashback. His legs were shredded and the bones appeared to be broken. He was flown to the trauma center in Baltimore. I don't know his outcome but he was alive when he was loaded onto the helicopter.

I have witnessed an accident which killed 8 on the spot and l was also injured, it was a human error,they didn't replaced the flashback arrestor after it was damaged.

Is it use one time only? Like a fuse?
If the flashback remains in the valve long enough to melt the spacer, yes, the spacer melts and seals the gas path permanently.

Is there a way to know that a Flashback have ever occurred?
I would assume the spring-loaded pressure valve would be in the triggered position from the heat and prevent the flow of gas.
