Every Derms’ Dirty Little Secret: The Skincare Product We Are Using Every Night

  • What is your dermatologist’s dirty little secret?
    Your dermatologists are likely on some form of retinoid as it is almost too good to be true. I have been on one for the last 8 years!
    We know that it has many benefits including:
    1. Acne treatment and prevention
    2. Anti aging where it helps with wrinkles and age spots
    3. Superficial acne scarring
    As a pill, this vitamin-A derived ingredient is an excellent treatment for acne: isotretinoin (aka Accutane).
    Use your retinoid (tretinoin, Retin-A, retinol, retinyl, tazarotene, alphaRet) at bedtime (sunlight will deactivate this vitamin A-derived medicated topical medication) and remember to stay patient and consistent to get results. Definitely consider incorporating this into your skincare regimen for acne and antiaging and know that you will not see antiaging effects overnight and know that this process is a marathon, not a sprint!!

    Update since filming this
    AlphaRet was exclusively sold out of dermatologists’ offices and I had to see a patient before they could obtain this. Now, people are obtaining online without seeing a dermatologist which is news to me. I always say to see your derm for this medication to review the side effects and precautions when starting on this (efficacy is similar to prescription strength tretinoin 0.025% in a study pubmed.ncbi.nlm.nih.gov/28762... ).

    Great video. 👍🏻 I have been using OTC retinoids for more than 25 years. I think I used the brand ROC not long after it came out. I recently switched to tretinion 0.05. I use the buffering method and I have had an easy time adjusting to nightly use. Those kids are cute 😀

    I am currently on tretinoin 0.05% mixed with niacinimide 2.5%. I started small at 0.018% in March of 2020. Now it’s been almost a year and I have seen so much change in my skin for the better. My skin always looked textured, bumpy, constant closed comedones all over the face. My skin is soft now. My acne is minimized. My pores are smaller. I’m so glad I started tretinoin when I did. I am 26 now but started at 25.

    I love tretinoin! Been using it for a year now (only wish I started in my 20’s) for anti-aging. It’s definitely helped w/ the fine lines around my eyes & overall smoothness of my face.

      Niomi Campbell's bad hairday channel thanks for watching! I would say not to do a scrub and a cleanser at the same time or on the same day, especially when initially starting on a retinoid. Would just stick to a BHA cleanser and the retinol. If this is irritating, hold off of the bha cleanser until your skin gets used to the retinol. Take care!

    Hi, you are doing shuch a good job.
    I like your show, just two days I watch your channel I have learnt a lot how to spent money my on the right and good product. I have been using Clinique cream, eye cream lotion, day cream , night cream and sun cream. However, I it is good, very expensive. Do prefer any other product just do the same hob but with resale price. I would like to mention that my skin is very sensitive and dry. Without using sun cream during winter I cant cook or go out because it gets red like it burnt.

    I'm 59 with no acne and very little wrinkles but I know I've got some sun damage throughout the years from not using sunscreen . I've recently bought ROC brand products to maybe make my face look more clear and skin pores will this help?

    Can you used that along with glycolic acid and benzoyl peroxide? Benzoyl peroxide seems to be the only thing that helps my acne….but glycolic acid seems to be helping texture and fading some dark marks- not all! Will retinoids further assist with the dark marks and texture?

    I’m using retacynl (tretinoin 0.025%) by mixing with my moisturiser in a 1:2 ratio. Does it reduce its potency a lot? But I’m still getting a bit of irritation and flaking even with this Low dose

    Hello Dr. Sugai! I recently started using alpharet and haven’t had any irritation. Is it safe to use every night ? Also, is it better to use other products before or after the alpharet? I use a perricone vitamin C serum and eye cream . What’s the best order and anything I should add?

      @@danielsugaimd Hi Dr 🙋🏻‍♀️ 🙋🏻‍♀️ I have some questions about retinols... after reading and learning of the amazing benefits of retinol I’d like to start using them on my skincare routine... I live in Australia and here we need a prescription for retinoids but I’m not sure how can I get it. 1. Do I have to ask my GP for the prescription or I have to see a dermatologist for the prescription?
      2. Can I start with the tretinoid lowest percentage or I have to start with another product like a moisturiser cream with retinol (alpharet)... ?
      3. At the moment we are in summer here in Oz and I love to take my kids to the beach, pool... it’s possible to apply retinols at night and be in the sun during the day (sunscreen of course), or should I wait til summer is over to start including a retinols in my skincare?.
      3. If my main concern are wrinkles around eyes, can I use tretinoid or alpharet around eyes?

    Question: Will the Skinbetter AlpaRet also help with acne? Is it as effective as Tret, I was prescribed Tret but it’s a little to drying and irritating. Wondering if I could switch and still get the acne benefits as well as anti aging.

      I love Alpha Ret. They now have Alpha Ret clearing serum. Essentially, it’s the Alpha Ret with a few added ingredients targeted for those with Acne prone individuals. Don’t apply close to eye area, I learned the hard way. Only use every other night. I haven’t had any irritation or drying.

    Hi, Dr. Sugai. At what age should we start using retinoids? I recently had a face allergy after using a sheet mask which caused redness, puffiness, and enlarged pores. I had an anti-allergy med for a few days until all it went away. However, I still feel there is puffiness around my pore area and maybe some loss of elasticity. Will incorporating retinoid help with puffiness and restoring elasticity. Thank you so much!!

      Do you have to get tret prescribed? I have had cystic acne since teens now 24 my derm rec I try accurate but I gave not I asked one time wat to do for scars she said differin now learning it takes time to see diff so now I’m usuin pea sized amount daily at night the place I go to each appt I saw diff assistant or whatever tellin me this but never got offer tret in cream but did try antibiotics pill benzoyl peroxide, clindamycin to no avail

      I tried that combo (olay max retinol) and it was ok for me... but I can tolerate LOTS of acids. DO NOT try glycolic with Tretinoin. Lol I look like a boiled lobster 🦞 😁👌🏼

    Hi Dr.Sugai, I just got the AlphaRet night cream today, Im excited to start using this, you said in an older video from last year that you checked with skin science and this is made with 0.1 tretinoin added with glycolic acid and lactic acid, wow it sounds great, do you still like it .thx.

      Alpharet doesnt have tretinoin, tretinoin is its own retinoid that you can only obtain from a dermatologist. Alpharet is a retinyl ester mixed with lactic acid.

      That’s what I want to know too. My friend gave me a tube of it because it was too strong for her. Mine is two years old. So basically our question is how long can it sit before it should be thrown out.

    I used accutane for body acne but my acne turned into keloids :( I unfortunately wasnt aware that I had the keloid gene so it was a bad surprise. Does this mean I cant use other retinoids on my face in the future? I’m afraid of using collagen too :(

      Don’t forget @lilychen that some have warnings on it not to use while pregnant so I would hold off on her using it till down the road after she has kids. I had a miscarriage while using it (one healthy pregnancy prior) Once I found out I was pregnant I obviously stopped but then miscarried so I wonder if it was still in my system. I used it to help with acne scarring. I was 25 at the time. This was back in the 90’s . Have her wait to be safe and like the Doctor said just have her use sunscreen while she’s young. She’ll have plenty of time to use it once she’s older.

    I love tretinoin but recently my eyes haven’t been closing all the way at night-leaving a sliver of dry eye. I’m putting an ointment in my eyes at night. Any chance I can ever use my tretinoin again without it traveling up and into my eyes. Any barrier I could use ?

      melissa pintone glad you are using Retin-A! I love my retinoids. TBH, I have never tried Dermarolling but I have become a recent believer of microneedling. I did a demo using RF microneedling on a few patients and on myself before the pandemic and was close to buying the device but with covid-19, that was obviously put on hold for sometime. I would not recommend Dermarolling or microneedling if you have inflamed skin or active acne. I should do a video on this soon. Thanks for watching and for the question! 🙏🏼🙏🏼🙏🏼

      melissa pintone we don’t have a particular device yet but we were demo-ing the Scarlet and Agnes RF microneedling devices which were great! I would love to add those to my practice. In the meantime, I do a lot of chemical peels which is effective for scars, pores and some tightening.

    I have had a difficult time using tretinoin and am at a point where I am thinking of stopping! I am on the lowest dose possible and it seems no matter how much sunscreen I put on and how often I reapply my cheeks are always red after coming in from a walk. Maybe some of us never get acclimated to it?

    I strongly advise against the use of trenoin because i got an autoimmune disease from it and have chronic kidney inflammation for the rest of my life.

It is scientifically proven that isotretinoin has immunomodulating effects that may induce some autoimmune diseases such as Crohn's disease, immune-mediated diabetes, and Guillain-Barré syndrome. Autoimmune disorders may be triggered by oral isotretinoin treatment. Clinicians prescribing isotretinoin should be aware of the possible association between isotretinoin intake and autoimmune diseases. I can gladly share you a lot of internet links to scientific studies and research papers proving the correlation.

    How do you know alpharet is prescription strength if the manufacturer does not disclose the retinol %. I believe it is a matter of ethics tgatbyoy clarify or retract your statement.

      Perhaps you didn't read my comment above. You stated the product alpharet is prescription strength. However the manufacturer does not disclose the percent retinol. Meaning, while a study can show alpharet as affective for reducing signs of photoaging, it's inaccurate to call it a prescription strength retinol. Ie just like to see a small amount of honesty and adherence to ethics from the medical community. Please explain why Alpharet is prescription strength given the % is undisclosed by the corporation. I don't want to bring this to the attention of a medical board if I don't have to.

      @@ra803g6 yes- the company does not disclose the percentage because my take is that it is a form of retinoid but not a “retinol.” If it did have a percentage, it is not comparing apples to apples. When I made this video, patients had to see me first before obtaining this as my office handled it like a prescription too. There was no way to get online at the time. Nowadays though, people are getting their hands on it online without seeing a dermatologist (I don’t know how) and so the “prescription” is no longer needed. I am using the term “prescription strength” based on this study saying that the efficacy of alpharet is similar to prescription strength retinoids like tretinoin. Thanks for the discussion. ✌️🤙

      @@ra803g6 i put an update in the description as things have changed since i filmed this. My impression before was that you had to see a derm to get this but now things have changed (I have been made aware of this recently that people are obtaining it online without seeing a dermatologist).

    When talking about retinol you MUST mention the percentage of retinol... -1% +1% retinol ??? else the basics of the review is quite useless.
