I left Internal Medicine to go into Anesthesia. As an new Internist, I was taken aback when patients were (30 years ago) super concerned with who their Internist was and they didn't even like it if I took a vacation. So one of the most refreshing parts of the field was the impersonality of it. In my residency, which was way before any one introduced themselves at the beginning of the case, occasionally the surgeon would ask me my name. I remember answering "anesthesia" to one particularly arrogant and obnoxious surgeon. In practice, things changed completely, and I knew all the surgeons, OR nurses and techs well and we all knew everyone's names. As our scope of practice has grown, and we are so spread out, I am more likely to know the surgeon than the anesthesiologist or CRNA who relieves me at the end of the day. Its a strange world!

The problem at least at the hospital where I work is the anesthesiologist switches out so frequently for breaks, that there is often another anesthesiologist than the doc that we started the case with there in the middle of the case… so sometimes you have no choice but to refer to them like that, although I often say “hey doc”, or have to do a reintroduction. But usually when you need the anesthesiologist’s attention in the middle of the case, there’s no time for reintroductions.

Hello Max! Just ran into your channel because I’m having surgery next week and although this is not my first rodeo I am pretty nervous. So glad I found your channel because you’ve helped me calm some of those nerves. I was wondering if you could do a video about the effects certain substances may have on anesthesia and the importance of disclosing the use of those substances. I’ve been taking hemp products that contain levels of THC and I had no idea the cannabinoids can stay stored in your fat cells for up to 90 days. How will this effect the out come of the anesthesia? I have stopped taking them and by the time I have my surgery, it would be 3 weeks since I’ve taken them.

Max, welcome to the world of unequal status in the OR. Surgeons rarely view us as equal partners trying to help patients get through surgery. Also, ‘hey anesthesia’ covers CRNAs & AAs in addition to (physician) anesthesiologists. As long as their patients remain safe n the OR table, for the most part, they are indifferent to who is administering the anesthesia. Hey, it sucks but welcome to the club. 😜

In instances where I myself have anesthesia I noticed a weird occurence. In instances where I was put to sleep with inhaled anesthetics like sevoflurane or isoflurane, I always puked. In instances where I was put to sleep withTIVA (Total intraveinous anesthesia) I've never even been nauseous. Maybe Dr. Max can explain or already has?

All Anesthesiology residency programs require 6 months of critical care training throughout residency and you can then follow that up with a 1 year critical care fellowship through the ABA. So, there's no point to doing a combined residency with IM unless you want to practice internal medicine - which is a bit strange if you want to be an Anesthesiologist.
