Yes, you can say the name in single strech. Also, in formal introductions it's better to introduce yourself with full name. Yess you can add mr. Mrs. If you are talking about others like 'das ist Herr Müller' as in 'that is mr. Müller' Also 'frau' is mrs. Yes, you can add Dr. As well
