Iam currently doing my internship, unfortunately ive failed in the anatomy in my first year(i did pass in the supplementary 2 months later). Does that matter while applying for usmle? Is it a red flag? Should i start my usmle journey or go with plab/amc/neet pg??

Is it okay to say the decision to become a doctor was not yours but over time you came to love it?
Not really. It should be your decision perhaps inspired by others.
Is it appropriate to say about family or fiancé in the reason of applying to specific program? Or it should be only related to program or it’s curriculum or any other thing?
Both. It is ok to talk about family member in the same program but that can't be the only reason. The program's clinicals didactics and other important points need to be stressed

What answer for failing step 1 once and are there chances for fail candidate for IM
Depends on other aspects of profile. If all scores including step 3 are lower vs. One off
