Examination of any swelling "lump"
1. Inspection (Site, Number, Shape, Size "approximate size" , Surface, colour , Skin overlying " dilated veins, scars, ulcers, pigmentation, previous incision, visible pulsations = absent, but there is site of needle biopsy in this case".
2. Palpation
2.A. Superficial Palpation (Temperature, Tenderness)
2.B. Deep Palpation (accurate size, surface "smooth and lobulated in this case", edge "well defined and slipping in this case", consistency "solid or cystic, cystic means that the swelling is yielding under pressure"
Bipolar Fluctuation test "to differentiate between pseudocyst and true cyst as in case of pseudocyst, the impulse is transmitted in the vertical direction only but in case of true cyst the impulse is transmitted in all directions"
Search for compressibility ( and reducibility in case of hernia)
= The swelling is soft, pseudo fluctuating and not compressible
Look if the mass is fixed to the skin (by pinching test) or underlying muscles
In this case the mass is mobile not fixed
No special percussion or auscultation in this case
Finally, we must examine all draining lymph nodes and search for distal effects of this swelling if present
Provisional diagnosis : most probably lipoma for further investigations ( as ultrasound and MRI)
