- The hepatocellular carcinoma is the most common liver cancer of which incidence is rising in the US. The common cause of this disease is non-alcoholic fatty liver, especially NASH (nonalcoholic steatohepatitis) as well as alcoholic liver cirrhosis.
When the cancer is liver-confined, patients can be cured by surgical resection or liver transplantation.
But when diagnosed in advanced stages, the prognosis was decimal in the past because traditional chemotherapy is not effective in this kind of cancer.
Thanks to recent advances in locoregional therapies and systemic therapy with immunotherapy drugs and multitargeted tyrosine kinase inhibitors, the survival rate has been imroving.

ERROR:
At 17:25, I wrote and said that the combination of nivolumab and pembrolizumab is used for the 3rd line therapy.
Pembrolizumab should be ipilimumab.
In other words, the combination of nivolumab and ipilimumab is correct.

My father was a well built 6 footer, just turned 65 a month ago. HCC was diagnosed with HCC in October 2022. Chemo was ruled out, surgery was ruled out. He was prescribed Lenvatinib capsules. Coped well until July. Became too weak for immunotherapy. Slowly grew weak and died peacefully on 07th October 2023. The hard truth is, once you have HCC, death is only prolonged, and whatever you do, it cannot be evaded. That's the hard truth!!😐😐😐😐
During diagnosed HCC spread any other organ??
@@meltoagee9999 No, that was the first thing the doctor did. Conducted detailed MRI and PET scan of the whole body, head to toe and spread was ruled out.
Hi why he was not given chemo or radiation treatment?
Also you mention he died 7 th October 2023 ? But he developed the HCC in October 2023 and cope until July don’t make any sense

For MRI scan of the liver, gadoxetic acid (Eovist) enhanced MRI is more sensitive than using gadolinium as a contrast because Eovist is taken up by hepatocytes, and excreted into the bile as well as in the urine.

My father has been diagnosed HCC last week, and he got immunotherapy using atezolizumab and bevacizumab, also a radiotherapy session, just for prolong said the oncologist, can't be cured. Is the patient will survive longer if he gets immunotherapy routinely? Are there any possible side effects of immunotherapy and radiotherapy for a 77 years old male patient? thank you in advance
It depends how well he responds to the treatment.
If he did, he may be able to live long with continuation of immunotherapy.

How ur father now?
My father has received 9 cycles of atezolizumab and bevacizumab and showed good result. In this early month, the CT scan evaluation showed there was a growth size of one nodule and the AFP was also increase to 40 mcg / L (usually AFP is around 20), so he had radiotherapy (1 session) and the immunotherapy agent has been changed to nivolumab single agent.

My mother has diabetes since 2002 and using insulin. She is also having blood pressure disease. Her age is 69. She is diagnosed with HCC in sep 2022 with 5cm size in segment II with AFP normal. She is Hepatitis B and C negative. As of sep 2023 HCC size was 8 cm. She underwent TACE in 5 oct 2023. Remained in hospital for 20 days because of high fever, constipation and body ache. IR radiologist says TACE went well. HCC size is 7.2 now. We will again do CT with contrast in FEB 2024. Now she is discharged but from past 1.5 months she is having uncontrolled blood sugar level getting high as much as 450 and low as 80. Her BP is also stabled just 15 days back. But she is having sever constipation issue and also developed piles. How you see this case sir. Your kind words may benefit her.

For advanced disease like Stage C by BCLC, systemic therapy is recommended. Please review my lecture.

Management of HCC is complicated. Your lecture is nothing but reiteration of book work, though comprehensive, almost lack of personal concept or recommendation, while some parts are outdated. The parts on PET and Y90 are confusing, if not biased.
