    CARE OF TRACHEOSTOMY
    Care of tracheostomy differs in patients who can do self care or in bed ridden patients who have to be helped with tracheostomy care.
    The basic principle is to keep the air moist as the natural organs that is the mouth and the nose which moisten the air before it enters the wind pipe are bypassed because of tracheostomy. Few tips to keep the air moist are
    1. Drinking lots of water if not contraindicated medically.
    2. Saline wash as advised by doctor.
    3. Humidifying the air in the room by humidifiers.
    4. Care of skin around the tracheostomy
    a. Special care of skin around the tracheostomy should be done in such a way that the water and soap does not enter the tracheostomy. It is best to clean it with sterile piece of gauze. Any secretion or mucus which gathers near the skin should be dislodged gently without disturbing the tracheostomy. Any redness or soreness on the skin should be noted and reported.
    b. If mucoid secretions tend to collect around the tracheostomy then plain gauze can be kept around the opening.
    5. Suction :- it is required time to time to bring out secretions from the lungs. In a patient who is able to take care of the tracheostomy himself it is easy for him to bring it out and the patient will know when to do suction if the chest feels heavy. In bed ridden patients the attendant has to be taught to do gentle suction as and when the secretions can be heard in the chest or when they are coming out.
    6. Few tips for doing suction
    a. Keep the machine off while inserting the suction tube.
    b. Don’t insert the catheter inside for more than 10 seconds specially in patients requiring oxygen.
    c. After insertion of the tube pull out tube a little before switching on the machine.
    d. Rinse the catheter with water or saline in between suctions.
    e. Keep the head up.
    f. Wipe out whatever secretions accumulate around the tracheostomy or in the mouth.
    These are few tips regarding tracheostomy care but ALWAYS CONSULT your Doctor or a senior nursing attendant before doing tracheostomy care by yourself.

    technically she contaminated her non dominant hand, which she was going to dirty anyway touching the non sterile suction handle. it definitely could have been a smoother affair with the sterile gloves. that dummy is totally unnerving!

    "Her breathing looks good her color looks good" I am just laughing so danged hard! Really if my patient looked like that after I suctioned her I'd be really worried. Something is wrong with Mrs. Jones. Something is very wrong.

      The patient would be dead by the time she gets round to it , the phlem /sercreations would be so far down the respiratory tract the cannular wouldn't reach it ... If a patient needs obstructional intervention glove up quickly grab everything and move as profienctly as possible. There's no time to lay everything out neat ... unless it's planned routine care

    Oh my God, she didn't even WASH her HANDS before or after the procedure. ALL NURSES PLEASE NEVER EVER FORGET TO WASH YOUR HANDS BEFORE AND AFTER TOUCHING A PATIENT.

    But you should always have non- sterile gloves on...especially when untying the trach dressing, mask, tubing etc...because very often there are secretions, sputum etc...that contain all kinds or harmful bacteria on the ties, tubing etc.., and you absolutely do not want to get it on yours fingers or hands....then just change over to the sterile gloves...that is what I do...
    PS...NEVER stand positioned directly in front of trach....especially uncovered.....make sure that you stand off to the side so that in event that the patient coughs while the trach is uncovered, to keep sputum from getting onto the face, neck arm, etc of the nurse providing the trach care......This is nice....a refesher inservice on line......Thanks for the video!

    She should have wear a mask when she put on the gloves. Who knows all that talking saliva splash on the sterile glove. And she didn't wash her hands prior and after the procedure . She broke the sterile field

    this doesn't even need to be a sterile procedure... teach this to patients and they won't do this at home if dc'd with trach... this is a joke.. Good for students tho.. but in the real world of nursing.. you'll develop your own technique.

    It's the proximal portion of the tube that needs to stay sterile.... proximal because it is closest to the body, not distal.

    I don't think it is a clean technique to stick clean applicators used to clean the stoma into contaminated NS that was used to flush the suction tubing between suctioning.

    bad demonstration,
    You need to work on your sterile field. you touched the gloves and contaminated yourself. Also when you insert the cath, it's important to mention you have to back up 1-2 cm if u feel any resistance, then start the suction, never more than 10 sec, wait 2-3 min in between. oh and 110-150mmHg plz.
