Adenomyosis vs. Fibroids: Why MRI Is the Gold Standard for Correct Diagnosis

  • At Atlanta Fibroid Center we frequently see patients diagnosed with uterine fibroids but after reviewing their MRI images, Dr. John Lipman sometimes discovers that a patient has adenomyosis or both, uterine fibroids and adenomyosis. It's very important to diagnose conditions correctly, but in case of adenomyosis it's not easy to do without MRI.

    Hello! Please tell me if the MRI has to be with contrast or without contrast, you can also see if there are fibroids or Endometriosis, Endometritis without contrast?

      For fibroids, no contrast is necessary. Detection of endometriosis can be difficult in many cases. The addition of iv contrast can increase detection.

    Unfortunately, one week ago I was scheduled for myomectomy after 2 MRIs, 2 CT scans, pap test, endometrium biopsy and blood tests that had indicated the presence of a very large fibroid (12 cm) but the doctor had to close me immediately after the first incision in the uterus due to massive bleeding. She provided me with 3 diagnostics: adenomyosis, degenerating fibroid or cancer. She said her inner voice told her to stitch me back in case of a cancerous fibroid and referred me to a different hospital for further tests in case of cancer. Is this normal? Thank you, Dr!

      I cannot comment on the specifics of your situation, but fibroids are very vascular and there is a lot of bleeding that can occur during surgery. It is one of the many advantages of UFE. You may want to get an opinion from an experienced Interventional Radiologist on your case; particularly with respect to the MRI findings. Good luck.

    Hi I know this is an old video but I’m just wanting to know if an hsg can pick up adenomyosis ? I got the hsg done last October and I just read the report yesterday. According to the specialist she suspects I have adenomyosis due to Venetial blind appearance to the myometrium. I really want to have children and I’m due to do more testing once i see my fertility specialist. What can I do ?

      Yes it can, but pelvic MRI a much better study to diagnose adenomyosis.

    Good day Dr. Dr my OB GYNE only made me do TVS. And all my OB gyne saw that i have adeno and endo. Do i still have to do MRI?

      If you want to be considered for Uterine Artery Embolization, most places require pelvic MRI. It is the best imaging tool we have (much higher resolution than ultrasound). Good luck.

    Is it possible to have adenomyosis and fibroids without any symptoms like heavy bleeding during period and pain?

      Yes. Most women with fibroids have no symptoms.

    Great Video !!! Thanks !!! Would adenomyosis and/or Fibroids cause a post menopausal woman to suddenly start bleeding/menstruating for 20 days ? Also, does heavy physical activity like lifting, exercising cause adenomyosis /Fibroids cause a post menopausal.woman to suddenly start bleeding ? Would a D&C along with progestin be an acceptable resolution in the above case ?

      The vast majority of people who experience post-menopausal bleeding are benign reasons but cancer must be excluded by your Gynecologist. That is usually done with sampling of the lining of your uterus but definitely discuss this with them. Good luck.

      Pregnancy after UAE is possible. Births after UAE are typically full-term and vaginal. While most women with fibroids have uneventful pregnancies, both fibroids and adenomyosis can cause significant infertility and every woman's situation is different. Good luck to you.
