What if we have thousands of images in a single folder ? It doesn't cause loading problems specially if we need to manage it in file manager in cPanel for example ?
That would be a large site, which means you have a system for managing images. If you’re using cPanel, the you can group the images into more logical folders rather than dates.
Is there any downside (SEO?) if in the 'Product permalinks' we use a custom base and instead of the default "Shop" replace it by something else? Product permalinks
What happens if you already have a site that you are developing and you change that setting? Will it keep the old files "as-is" but allow you to not keep compounding the problem? This is a great alternative to having to use a "media manager/folder" plugin
I don't know much about WordPress security. But there are plugins you can use e.g. WordFence etc. I'm sure there are a couple of videos about WP security on CZcams.

Yes, the old files will remain as is, only new uploads will be affected.
