Will they ever ask someone who is already collecting benefits for this form? Luckily I have copies of my original forms. I had them scanned in on my computer so I could make digital backup copies of them. I even kept all of the denial and over payment letters along with copies of written testimony from my teacher. I don't know how I would get another copy of that from the SSA if I ever lost them. Maybe I would get lucky and my attorney would still have them. I sure hope she does. They may be in a dusty box somewhere burried under other boxes of papers but hopefully she has them. I think what I have is probably a copy of all or most of it anyhow. A little advice, get a cheap used scanner from eBay and scan them yourself, it can be expensive to go to a service to get things digitized and hopefully there is no copy of it lingering on a hard drive in the machine that no one ever sees. That is right, some photocopiers had hard drives in them with copies of everything that was ever copied. It is unnerving really!!

do you have to fill out the function report if already disable for 20 years thank you
how fill out with mental health. the answers dont make sense when its mental health

Why didn't you start with question number 7?
The same principals apply when considering all the questions. If you view Part 1 and Part 2, you should have a good idea of why the form is asking what it's asking, and what to consider when writing your answer.
