    Ciprofloxacin also has coverage for pseudomonas (UTI and otitis externa). Doxycycline has some coverage for MRSA. Thanks for the great review!

    Vancomycin should be given orally when treating C.diff (not IV)

    What an excellent review... thanks so much! Might be worth mentioning the risk of Fanconi syndrome in expired tetracycline. Also, I have known cipro to be effective against pseudomonas. :)

    Indeed there is a fifth G cephalosporin ( Ceftaroline ) that covers MRSA .
    Another note : Cilastatin is not beta lactamase inhibitor , it is an enzyme inhibitor that impairs imipenem metabolism in renal tubules to increase its half-life .

    Great stuff....but one small note (as a pharmacist ;)).....cilastatin is actually combined with imipenem so that it's not degraded quickly in the kidney by dehydropeptidase....cilastatin inhibits this enzyme....it actually doesn't have antibacterial activity....great lecture! I'm getting ready for my boards and enjoy hearing the clinical perspective that as a newbie, I lack! ;)

    You might need to add the treatment of mastitis to your Doxacyline - drug of choice list, and thanks for the informative presentations you always provide.

      Actually in breastfeeding mothers we wouldn't want to use doxycycline, because of the side effects on babies and children under 8, cloxacillin and keflex are better choices for mastitis in this case.

    Amox-clav is useful in ear infections if does not clear with amoxicillin alone (e.g., caused by a beta-lactamase strain of H flu, S pneumo, or moraxella).

    18:18 Cilastatin is not a beta-lactamase inhibitor but instead inhibits the human enzyme dehydropeptidase I in the kidneys, preventing premature degradation, thus prolonging action time

      I have known Antibiotics for a long time. I read about it on the website medlineplus.gov/throatdisorders.html I used it to treat my throat (2 pills for 5 days) and my throat got better on the next day. Then my doctor prescribed me Antibiotics for the treatment of bronchitis. Recently, I gave the pills to my son. I bought them at the pharmacy vaigra.top/antibiotics-medicine-usa.html at an affordable price. My son used it for the treatment of bowel disorder. On the next day, diarrhea has gone. The pills are cheap and not addictive.

    Silver sulfadiazine is used as a topical for skin burns and wounds, but contraindicated in late trimester of pregnacy and babies less than 2months old, due to increasd risk of kernicterus, (bilirubin protien binding is affected by this drug)

    It was mentioned that vancomycin "IV" is used for the treatment of C. Diff diarrhea but correct me if I'm wrong don't we use vancomycin "PO" in this case? Thank you for the lecture!

      PO is correct. There should be an annotation on the video correcting this, but CZcams disabled them so I’ll have to find some other way to edit the video.

      @@pwbmd , I enjoy your lectures. Black box warning for Fluoroquinolones is increase risk of tendinitis and tendon rupture. In your slide, you have it as LOW risk.

    "I think that's kind of gross", hahahaha, totally cracked me up. I didn't see you were going that way with that sentence, I was sure you were about to describe the smell of C. diff diarrhea.
    Also, how do you differentiate C. diff diarrhea from diarrhea that is a side effect of another antibiotic?

      Dharin Sawh Technically yes inasmuch as there will be WBCs in the stool. But typically when people refer to dysentery, they're talking about grossly bloody stool along w/ mucus.

      Pravesh Raj Niroula Only after standard topicals (e.g. clinda/benzoyl, erythro/benzoyl) have been tried. Wouldn't be a bad idea to suggest a urine pregnancy test for girls and women because of its teratogenic potential before starting. Absolutely must get a UPT before starting tretinoin.

    So for kids under the age of 8 with lyme disease...do we use ampicillin or amoxicillin?...The 1st slide said ampicillin...but late you kept saying to use amoxicillin.?

      Agreed. These videos all originally had sound. At some point when CZcams updated their software, the audio was lot in many of my videos. It's been an exhaustive effort to track all of them down and re-upload them, but I'm working as quickly as I can.
