      This just happened to me. All my bloodwork came back normal, my husband's SA came back normal. I asked my doctor if we could try clomid, she said no. She just thinks our timing is off, although it's been almost a year of trying. I laid in bed and just completely sobbed because I know theres more options, but they're all so expensive and I feel like I'm not successful at being a woman because I'm struggling to conceive. We want one so badly and I'm about to turn 30 and it just keeps looking less and less likely.

      @@CrispiePotato I’m sorry. Yeah it’s not easy. Even though we don’t mean to, we end up comparing our fertility and TTC journey to others. That was my biggest thing I had to get out of my head. It took me a while to figure it out. This month I stopped tracking and taking tests because I was obsessing over it. I tried that this month and it’s helped me so much. It feels like a weight is lifted off my shoulders.
      Have you had an HCG test I believe it’s called. Where they use a catheter and some dye to see if your tubes are open and working right?? My doctor recommended I do that. She said it’s pretty painful. Reason being I had an appendectomy when I was little and my appendix ruptured so there was infection in that area. Well I guess you can develop scar tissue due to that, and my doctor said it very well could be that one tube is blocked.
      I actually just made a short video on my YT channel. It’s about the TTC journey and what I’ve experienced. Don’t worry you will be i my thoughts and prayers! Also give me updates! Baby dust ✨✨✨✨✨✨ ✨❤️

    Thank you, dr. Crawford, for the very useful videos!!
    I was wondering if there is any risk of cancer, caused by fertility treatments/procedures you have mentioned in this video?
    I am 34, probably facing unexplained infertility (not sure if slight higher prolactin levels might be a cause?!) no issue with the pituitary gland... I have been recommended to try last month a ‘natural’ approach, with letrozol and gonadotropin, my dr saw the 3 eggs at ultrasound, indicated when to have intercourse, but no success. I will have my tubes checked next month, but what else to investigate here??

    • @NatalieCrawfordMD
      @NatalieCrawfordMD  Před 3 lety

      Fro unexplained infertility, letrozole and gonadotropins do not increase success rates at all, i wouldn't recommend spending money/time on that. So ask your doctor clearly how to achieve a higher success rate.
      Cancer risk - no increase risk seen with fertility treatments until women have done 8 egg retrievals, and even then it is a very very low increase in borderline ovarian tumors.

    Hi Dr Crawford! Thank you for this video. We went through two rounds of retrieval’s with no embryos. Retrieved 38 and 26 but arrested before blastocyst phase. Mostly likely my egg quality. My REI thinks that I should try IUI before attempting a 3rd retrieval since I did naturally get pregnant 3 years ago But is that worth it if the chances are so low?

    • @NatalieCrawfordMD
      @NatalieCrawfordMD  Před 3 lety

      If egg quality is the issue, why would an IUI help? I see this - and it can be overcome, make sure you have a good cycle review with your doctor and see what changes could be made (supplements? protocol? hGH? sperm?) and consider getting a second opinion. See the IVF tips YT episode and I have a podcast episode about IVF failing that may help give insight.

    I definitely wish I would have had all of this information before wasting almost an entire year doing multiple IUIs. I do have a question about my unexplained infertility. I have had multiple egg retrievals and it always looks like I have lots of good follicles (good sizes) but less than half of my eggs that are retrieved are mature. For example during my first cycle I had 21 eggs, but only 9 were mature, and even less fertilized. The same thing happened during my second retrieval. Have you seen this before? During my second retrieval we added a growth hormone and there was no difference.

    My husband and I were diagnosed with unexplained infertility and it really was frustrating to hear. We had so many tests done just to find out we were both totally normal and there was no discernable issue to address. We only got our diagnosis at the end of last year and we were supposed to start IUI in the spring, but because of COVID that didn't happen, and then we decided to put it off because I didn't want to be pregnant during a pandemic. Our doctor is quite confident IUI will work for us though, so when we are ready to get pregnant, hopefully it won't take too long.

I got pregnant February 2019 and had a miscarriage around 7 weeks (my first ever pregnancy). I’ve had blood work done, my tubes have been checked and are open and I’m a healthy individual. I plan to go see my OB in January for next steps. IF you have ever been pregnant before can you still be categorized as having unexplained fertility? And would IUI + clomid be a good place to start (in general because I know everyone is different)? Thank you!

      This depend on family planning goals. Overall - conception in the past is a positive sign for future success, but not guaranteed. We even see secondary unexplained infertility which is really difficult. Hard to know what was good luck (was a miscarriage cycle just hitting the 2-4%?). I like your plan to see your OB and make sure you are getting everything tested (sperm, ovarian reserve, etc) and then consider a fertility specialist for the best counseling. Hugs!

    Currently going through infertility treatment now with DOR. Unfortunately IVF is not an option so we did one Clomid/Ovidrel/IUI cycle that wasn't effective. We plan to try again in the new year. What would you say is the average for having it work under optimal conditions?

    • @NatalieCrawfordMD
      @NatalieCrawfordMD  Před 3 lety

      We stim DOR often - depends on multiple factors, but many of us don't consider that an exclusion to IVF. If unexplained infertility, average success rates are 5-10% for clomid IUI. Not 0! But always consider a second opinion if you need one.

    My girlfriend has pcos but she was ovulating on her own when she lost weight. But she would always ovulate around day 21. She taking clomid now and ovulated on day 14 or 15. Is it better thats she is ovulating at a more normal time or doesn't matter? We've been trying for somewhere around a year and a half. Tried femara last year and it didn't work. First month of clomid and no success.

    That's my official diagnosis, but I also have PCOS. My RE explained that since all my cycles are ovulatory and generally a reasonable length (around 37 days), she considered our case unexplained. Is that a common thing?

    • @NatalieCrawfordMD
      @NatalieCrawfordMD  Před 3 lety

      Yes I say this too - PCOS can "graduate" to unexplained infertility after 6 ovulatory cycles and no conception (meaning, ovulation is no longer the rate limiting step)

    We have been trying since July 2018, and have had two miscarriages in that time. Me and my husband have had the testing done and everything has come back normal, except I had higher levels of testosterone (I have been taking myo inositol and d chiro inositol and it has lowered it to normal range, no other signs of PCOS). I get monthly periods and positive OPK's monthly so I would expect that I ovulate monthly. The only thing I'm concerned about is low progesterone but my OB and fertility doc don't believe that it does anything. I had my progesterone tested once 7 days after a positive OPK and it came back at 9.6. I have been taking femara for 6 months with no success. We planned on doing IUI for 3 months starting in January. My insurance covers nothing fertility related so IUI and IVF would be out of pocket. Would it be worth our time to try IUI+gondatropins since femara alone hasn't been working for us.

    • @NatalieCrawfordMD
      @NatalieCrawfordMD  Před 3 lety

      9.6 is a normal luteal progesterone!!! The point of a luteal progesterone level is to confirm ovulation and anything over 3 ng/mL is normal. Femara and IC doesn't add anything to your success rates if you are already ovulatory, so this is complex - have these cycles been monitored? Are you getting 1 egg or more? Ask your doc. To me, if femara achieves our goal of 2-3 eggs then there is no reason to up the cost/risk with FSH for most patients.

      @@NatalieCrawfordMD I'm on the highest dose- 12.5 mg days 3-7. I have had two monitored cycles and I've had 2 eggs. I have not used a trigger shot. I don't know if that would make a difference.

    Do you recommend regular IVF or IVF + ICSI to try first for unexplained infertility?
    We are unexplained except that my husband’s SA came back with low morphology at 1% twice. The other parameters are all good, so my doctor sees his result as normal. I went to another doctor to confirm, and he agrees the same. We have not found any other reasons why we are not getting pregnant, so I guess we are unexplained infertility :(
