Emphysema - Clinical Presentation, Diagnosis, and Management

    . Emphysema is abnormal, permanent-dilatation of the acini or respiratory lobule with the destruction of the fibrous septa, without apparent scarring or fibrosis.
    In this video, Dr. Mobeen Syed discusses the clinical manifestation of emphysema.
    The patients of pan-lobular emphysema (pure-emphysema), may have hunched-over back with dyspnea. The patients would have long expirations and a loss of weight.
    They do not develop congestive heart failure.
    Cough and wheezing are not present in pure emphysema.
    The patients of pan-lobular emphysema suffer from dyspnea. They have enlarged lungs and perform extra ventilation, which minimizes the risk of hypoxia. They are called pink puffers.
    On the other hand, the patients who suffer from both emphysema and chronic bronchitis do not develop dyspnea. They have reduced gaseous exchange due to emphysema, which makes them cyanotic. They have hypoxia and hypercapnia.
    They are called blue bloaters.
    They develop cor-pulmonale due to pulmonary hypertension.
    Treatment approaches to emphysema include steroids, avoidance of smoking, phosphodiesterase-inhibitors, and anti-inflammatory drugs.
    In the patients of emphysema, the principal causes of death are respiratory-acidosis, coma, and right heart failure (due to pulmonary hypertension).
