Easy way to think of it is that President is the Head of State while the PM is head of government. The UK for example is a Constitutional Monarchy, therefore their Head of State is King Charles.
Portugal is a Republic and also has both. President is usually the highest authority. Prime Minister is the top official in government administration and leads the cabinet of ministers
