What are your comments if you see a black streak reflex in distant direct ophthalmoscopy when patient see towards you and reflex disappear when patient focus on far target . Age of patient between 40-50

Media opacity may be there. Do one thing, ask the parent to fixate at your finger. Now keep you Retinoscopy position fixed and Move your finger right and left. If black streak move with the finger then it's corneal media opacity and if black streak move against the finger then it's lenticular opacity.

Please consult with a retina specialist to check your retina. Then go for your Binocular Assessment to an Optometrist. Vision Therapy may improve your condition.
