To get to mega satan, you have to pick either the negative or the Polaroid, then make sure you have gone to two angel rooms and bomb the statue, kill two angels, get both halves of the key. Next, get past either satan, or Isaac, and when you reach the dark room or the chest, the big gold door will unlock. Inside is mega satan. The catch? good luck beating him.

DopClanFoo You CAN NOT assume how much someone plays video games, there people who play 5 strait hours nearly every day, and some people who play about maybe a half an hour every other day. I am a person who plays 5 hours strait, but I didn't for the binding of Isaac, and for the first LONG time for me playing the game I NEVER looked anything up online so I, FUCKING, SUCKED, I think I killed mom like twice and Satan once only because I used azazel on almost every run after I unlocked him (like any noob would). I've gotten as far as golden god now and have found most of the items in the game, found most of the secrets, and unlocked about a sixth of all the characters unlocks (the ones you get for beating those milestones bosses on hard mode.) I played the game for like 2 weeks without every looking stuff up online, didn't play the game for a long time after that and then I dove back into it played it for a long ass time now, took another break, and now I'm on a rush for the real platinum god because I want to get it before I install afterbirth because all those new items and secrets and such are just going to confuse the HELL out of me when trying to get those last few items for the real platinum god mixed in with the hundreds of other ones, and other secrets that after birth will come with. besides with the HUNDREDS of items already in the game, especially ones with confusing names like "Polyphemus" I bet even players with hundreds of hours in the game couldn't spell most of the names of most of the items in the game correctly. and as tradition continues, and only for that reason.... idiot
