How do you know where to put the shirt like, to make sure it is straight?

You can mark the shirt before you hoop it. It is best to use an embroidery hooper but it is not necessary.

How did you digitise?

You can digitize the applique in your embroidery software (no cover stitch) and using the minus front and back of your converted vector file create the knockout effect. Duplicate the embroidery file before you make the vector file. Or using the same fonts make a knockout file in your design software. I use Corel Draw. Then digitize the design in your embroidery software. I used Wilcom.
