      Dizziness, vertigo, "off balance" issues with split second "head jolts" (ENT and Neuro have no idea what is is after Neuro did brain scan, tests, etc), EXTREME fatigue, muscle aches (now in my knees), terrible sleep patterns, worst mood swings, swelling/edema, heart arrhythmias (all tests came back clear, aside from benign PACSs/PVSs), night sweats for past decade (occasionally), very irregular cycles (I'm on day 56 w/o period, longest stretch yet (used to fluctuate between 26, 23, 28, 25 days, up to day 39), so definitely in late peri. My doctors refuse to do extensive hormone testing, and just want to put me on an SSRI. No thank you. It's their way to numb you up and send you on your way. They tell me that they are not allowed to put me on hormones (they won't even test me!) unless I've gone a full year without a period (which I know is nonsense and unethical of them). It's been debilitating for a long time, and I'm awake at 2:50 AM as usual. Normal sleep and a normal life as I knew it is completely gone. So is the quality of life. I'm almost 52 and have been suffering with these issues for nearly a decade, but they got bad in 2020, and the worst yet in the past nearly 2 years. Keeps getting worse. If you can offer suggestions, or tele-health appointments, incl testing, please reply. I'm in Southern California.

      So, basically all of the symptoms I've mentioned are the worst to deal with. They keep bouncing me around to specialists, putting me through further testing, more appointments, more scans, and everything checks out okay. They refuse to talk with me about my hormones, and they definitely aren't helping whatsoever. It gets ignored completely when I bring it up. I'm frustrated to say the least. It's unethical how they allow women's quality of life to suffer to such extremes.

      I am 45 and I have an IUD, this is my 2nd one. The first one I had a period for 2 months then period stopped. I got my 2nd one in April. I have had a period every month. For the last few months I have sore breast on and off all month with spotting through the month. I also gave heart palpitations, mood swings, and night sweats. My dr said since I have a steady period every month I am not perimenopausal and thinking I need antidepressants. I have suffered from exhaustion and brain fog for the past 3 years as well. Could the IUD be causing these recent symptoms?

    Just an aside. I am in peri menopause and actually a patient of doctor Hirsch. The hormonal effect on sleep is very real. But I wanted to add another wrinkle to the whole sleep thing. I didn't realize over the years that I had become more and more sensitive to caffeine. Only when I started getting little heart palpitations and got checked out did a doctor suggest that I take a break from all stimulants just to see if that would make a difference. I literally only had one small cup of black tea every day, I was not a caffeine junkie. But when I gave up caffeine for a while, the palpitations stopped immediately. What I didn't expect was a huge change in my sleep. I was back to sleeping like a 16-year-old. I periodically allow myself a cup of black tea when I'm just really missing it, and invariably I have a terrible time going to sleep that night and will wake up feeling exhausted.
    So this is just another thing that women in midlife may potentially be dealing with in addition to the hormonal stuff. Apparently everyone becomes more sensitive to caffeine as they get older. Just another wrinkle to add to the mix. If you are someone struggling with sleep, maybe just try giving up caffeine for a month and see if it has any effect

      Same goes for alcohol and anxiety. My friend and I developed strong sensitivity to Caffeine or alcohol even in little amounts in midlife. It triggered increased anxiety, insomnia, and low mood. When I stopped all I felt much better

      Very true! Also the acidic measure in coffee...I think it gave me leaky gut. Stopped cold turkey and trying to heal my gut. My sleep did get better but I need to take supplements to help me sleep through the night.

    She used to teach at Harvard medical School. And she was the director of the Brigham and women's hospital midlife clinic. Then she went to midi health. And now she is in private practice.

    At the age of 48, I adhere to a regimen of 200 milligrams of bioidentical progesterone orally, supplemented by a modest application of testosterone cream. In contemplating adjustments to my hormone replacement therapy (HRT), particularly in the context of perimenopause, I inquire about the appropriateness of introducing a minimal dosage of estrogen. While my current HRT regimen has previously yielded satisfactory results, recent manifestations of low mood, dry skin, irregular menstrual cycles, and diminished libido prompt this query, despite the previously effective influence of testosterone.

    I just turned 45 and can fall asleep but I can’t stay asleep. Hot flashes and night sweats are starting. I toss like a rotisserie chicken and I can’t live like this for the next ten years 😭

      My insomnia is severe but I am trying a few natural remedies like Gaba, melatonin, 5htp, magnesium and a female supplement with plant estrogens. I will let you know how they work.

    I thought we only made progesterone 2 weeks of the month? What is the reason for no sleep the first 2 weeks of cycle?

    Yes, but what about women in the advanced stages of perimenopause who still struggle with insomnia, despite using bioidentical progesterone and a small amount of testosterone gel? Despite adhering to all recommended practices, nothing seems to alleviate the problem. This critical issue is not receiving the attention it deserves in current discussions.

    How do I get my HRT balance right to achieve the closest thing to a pre- peri menopause reset and get rid of this sudden onset of excess stomach weight? 😩
    I just want my body to process and metabolise how it did before peri menopause. This menopause mid section sitch is costing me a fortune in new clothes! ☹️🤭

      High fibre diet... Lots of cruciferous vegetables and good quality protein. At least 30gms of protein with each meal.

    🙏Ty I’m almost 42 and deep in the throes of perio and my anxiety is off the charts. Hot flashes and dizziness, 😶‍🌫️ just very low quality of life and I’m normally a very active person. My anxiety has turned into full blown panic and I’m worried about side effects of hrt and was confused about taking it for perio because I thought it was just for menopause.

    Hi Dr H, Thank you for your time and videos. I started estrogen and progesterone in lowest dosage 4 weeks ago . First I used the spray I felt little better little bit more energy and happier but I scared to transfer for my cats and teen son then I switch the patch. OMG with the patches first 2 days I am so very very angry to the point I can't do it anymore and I had to switch back to spray. I spray and cover my arm 2 hours so nobody gets in but then I am having a different problem my skin is reacting badly to the bad aids (not the med) getting really red itchy and sore.. I wish they had daily patches not weekly. Do you have any suggestions for me. Thank you so much.. Ps; I am also worried I take shower or wash my arm off the hormone, can anybody still get in them , my children taking baths at the same tub..

    Do any of you have painful HOT FLASHES!? It's in my legs and its like electricity running through them. I've told my Dr's and OBGYN and none of them know about it. I stopped having periods 5 years ago. I know exactly when because i had my last period during my mom's funeral, and I had almost gone a whole year before that. I would say the painful electricity part started the same time even though I'd been havinghot flashes for a long while. My Dr FINALLY gave me prempro about 2 years after I stopped, I was going insane from lack of sleep. It worked great until this April i started spotting on and off for about half the time for the next 5 weeks. I had to go off the estrogen until I got a Biopsy and it came back negative. The OBGYN said I could go back on prempro but I have been back to having painful electricity running through my legs. It only happens WHILE im have a hot flash. Weird huh? It should go away again once the prempro kicks back in. Also I get really cold for about an hour BEFORE a hot flash. Also unusual.

      I have been having this same strange symptom as well. It comes and goes and I think it's nerves related. I am praying we find the answers we need. This is no way to live.

    Does every single woman have to suffer this horrible stuff just because she's??? Does life really punish every single woman just because of aging something that we can't control don't consent to??
    I'm not trying to sound whiny but it really is unfair that this stuff even happens 😭 going through puberty with so much easier we didn't need any creams hormone replacement stuff and we were still young and beautiful not deteriorating 😭😭😭

      I have to say that this is the very first time that I have seen a woman besides myself blurt out the most truthful feelings about menopause and it’s horrible-ness. I thank you. I don’t have the same desire to off myself but I have been suffering in this meno misery since my mid 40’s. No everyone has the chemical makeup to be able to slap hormone patches or creams on themselves and be all hunky dory after. Some of us cannot use horse pee hormones due to many risks. I wouldn’t touch that crap with someone else’s body. That being said. We still feel awful and just want it to stop. I have had a lot of luck cutting sugar out of my diet, alcohol and processed foods but I get on and off relief from symptoms and then, like a wrecking ball, it all comes back and slams me down. Not all women get it like this, obviously, because of all women did they would all be bi****g and complaining like we are. I am right in the middle of a rollercoaster relapse right now and let me tell you, It’s bad. I won’t get into detail about the hot flashes, burning skin, insomnia, vertigo, anxiety, mood swings, Achebe’s, joint stiffness, peeing a lot and a host of other lovelies. I just wanted to let you know that there are a lot of us who suffer badly with it and it, it sucks, it’s unfair and it’s miserable! I acknowledge that!! Try Kari Anne Wright’s channel. Lots of humor but definitely more understanding rather than the 👀. Check it out!

    Does Mirena iud progestin work the same as the progesterone your talking about? I have that IUD but also think I may be experiencing perimenopause

      No, IUD's are like birth control. They're used for that purpose normally. IUD is synthetic progestins & NOT Micronized Progesterone USP. Synthetic progestins and synthetic estrogen have very high risks for stroke, blood clots, heart attacks, cancers. You want Estradiol and Micronized Progesterone USP only. Preferably Estradiol patches which pose even less risks. Creams have less absorption. Inserting Micronized Progesterone USP capsules can work too but the absorption is not as strong as taking it orally. Some people have problems swallowing pills of any kind for various health reasons so I've been inserting my Micronized Progesterone USP capsule vaginally right at bedtime. The gelatin capsule melts & the vagina expels it. That way as I'm sleeping the Progesterone is being absorbed. It's working!! It's all working & I'm already feeling & seeing benefits after years of suffering & especially the last 12 months have been very very scary & hard for me. I'm 49 years old. I currently use Estradiol patch 0.025 & change it twice a week & apply the patch to upper buttocks alternating each butt cheek with each patch change. Better absorption on the upper butt cheek. My Micronized Progesterone USP capsule is 100 mg. But again my body is not absorbing 100% of that since I'm inserting vaginally. I'm getting ready to get Micronized Progesterone USP through the "Troche" method (lozenge you put in mouth & take orally) since I can't swallow pills due to neck cancer twice & surgeries. This is done by a compounding pharmacy & made with pure Micronized Progesterone USP powder & NO additives or fillers!! The capsule is made with food coloring, titanium dioxide which is a carcinogenic, peanut oil & other fillers. And this is what we're swallowing or inserting vaginally!! Compounding is not bad like these docs make it sound. I would not go the "pellet" route though. You can't take pellets out once they're in & so therefore you cannot adjust dosages of hormones.

      Yes, the Mirena ID can be used as the progesterone component of HRT. So you could easily add a patch or whatever you prefer and not have to worry about taking extra progesterone.

    Dr. Hirsch, I had a partial hysterectomy 12 yrs ago and a recent unilateral oophorectomy. I began estrogen replacement therapy (transdermal patch) about 1,5 mos ago. We're still trying to get the dose right. A lot of my symptoms got better but I still have trouble falling asleep. Would it be reasonable to augment estrogen w/ progesterone?

    Im 40. Im working with a hrt doctor. He has my testosterone high and my progesterone 300mg. Still have a lot of issues. My estrogen is low and i feel like im going crazy😢. I know my estrogen low.its at 23

    Can you take Prometrium if you also have a hormonal IUD? Or is that too much progesterone? Also, do you need to match dosages of estradiol/progesterone? Meaning, if you get a higher amount or estradiol, are tou smart to get a higher dose of Prometrium too? To balance the two? Thanks for always giving us such great information!!

      I am not an expert but yes, from what I have learnt from many videos is that you do have to balance the progesterone to the estradiol, unless you are using an IUD with progestin.

    Can I take micronised progesterone 100mg (Utrogestan known in uk ) orally every night without estradiol ,Is it safe please ? Any advise would be great help. Many thanks!!

      My doctor said progesterone without estrogen is perfectly fine. In fact she won’t even offer me estrogen at 41 since I still have a regular period but she’s putting me on testosterone and progesterone

    Hi Dr, hope you are fine and wel. I had Bso, hysterectomy, Appendectomy and omentectomy in Dec 2022 for high grade serous cyst adenocarcinoma both ovaries but my Dr didn't prescribe any HRT. I'm obese with 26bmi,hot flushes, increase sweating, palpitations and occasionally high blood pressure. Please advise what should it do. Regards

    My dr has me on the compound , I tried 100mg pill last year and woke up freaking out and felt like I was going to pass out. Do you think that was the pill ? Discouraging to think the compound doesn't work. My sleep is terrible ! Did the oral hormone test and showed my progesterone was a 41 which is low. Any suggestions

    I was started on birth control to help with my perimenopausal symptoms. I'm the 3rd type all the way around symptoms. I'm only on day 6 of birth control as of 7/30/23 and wondering how Iong would it take to help or if I'm on the wrong method.

    I have a a question not on menopause but on estradiol i have been diagnosed with vaginal atrophy, had a hysterectomy 28 years ago a complete one one dr. Put me on estradiol, and i breakout, cant breath, skin peals, arms get bad eep sores, am i allergic to this med?

      I understand this just means the dosages need to be tweaked. I’m 61, on both progesterone and the estrodial patch. Did not have any bleeding. Mention this to your HCP.

    Geese …. I hate this perimenopause crap. My issue is being tired ALL the time. (No anemia or other issues) also …. Spotting a lot, negative for anything going on.

      She was faculty at Harvard. She taught the medical students about menopause treatment. This was when she was also the director of the midlife clinic at the Brigham and women's hospital in Boston.
