Can BALF (high counts of inflammatory cells like macrophages/eosonophiles) be also a diagnosis for ARDS?
Bronchoalveolar lavage fluid with a high count of inflammatory cells like macrophages and eosinophils can suggest an inflammatory process in the lungs, but it is not specific for ARDS. ARDS is typically diagnosed based on clinical criteria including acute onset, hypoxemia, bilateral infiltrates on imaging, and the absence of left atrial hypertension. While BALF can support the diagnosis by ruling out infections or identifying particular patterns like diffuse alveolar damage, it's not solely diagnostic for ARDS.

how patient is developing hypotension. please explain
Most likely due to the pneumonia causing sepsis and patient having septic shock
1. Hypovolemia: Inflammatory responses and increased capillary permeability in ARDS can lead to fluid leaking into interstitial spaces, decreasing blood volume and causing hypotension.
2. Sepsis and Septic Shock: ARDS often complicates sepsis, where massive inflammatory mediator release can cause blood vessels to dilate, leading to a drop in blood pressure.
3. Cardiac Dysfunction: Inflammation and hypoxia in ARDS can impair heart function, reducing cardiac output and blood pressure. Additionally, high positive end-expiratory pressure (PEEP) during ventilation can limit blood return to the heart.
Hypoxemia & Hypoxia have two different symptoms. Generally, hypoxemia will be accompanied with faster vitals such as increased HR, BP, & RR. But, as time goes on Hypoxia will occur leading to decreased BP, HR, RR, & cyanosis.
What occurs in ARDS .... respiratory alkalosis or metabolic-respiratory acidosis ?
