New Obesity meds? Could Ozempic or Wegovy no longer be the King of the Hill?

  • Nothing lasts forever.
    Especially in the world of drug development. It seems like just yesterday Ozempic/Wegovy gained approval for Obesity management. Wegovy is Ozempic; it is just used at a higher dose of 2.4mg once weekly.
    A new medication from the drug company Eli Lilly called Tirzepatide will be seeking FDA approval for diabetes management in 2022, and I suspect it will be seeking approval for obesity management shortly thereafter.
    Trials have been completed, and tirzepatide has been shown to have some very remarkable results not only in managing blood sugars but also in weight-loss.
    Let’s take a look at the results and see if the King will still be standing!

    thank you for sharing. I had a terrible reaction. Ended up in the ER, and the doctor tried it again, and I was so ill again. She pushed it on me so hard, and all I could think was "Obviously this is what everyone is using and there is nothing else out there!" In my Rheumatic Arthritis group, people had one of two results. Huge weight loss and happiness. OR, miserable, feeling depressed because it can make food not only now a non happiness, but almost everything else also. If you could not be on the drug, then most of us had to deal with huge depression as the "Cure" or "The thing that actually works" doesn't work for us. Some became suicidal. Others that had been successful also left, as they felt it was the DRUG and their working hard with the drug, dealign with some nausea and also choosing not to eat as much, or much at all, and we were NOT GIVING THE DRUG THE HELP IT NEEDED. It was our fault which for a support group was not supportive. I was vomiting, couldn't keep water down, was admitted for a couple of days to give me hydration as the one week shot works a week....but people are being rather mean. One thing is I understand this is a break through drug, but if it doesn't work for just a small portion of those that need it, well that's not a big deal. But for that small portion, we're miserable.

    This encouraging. Hopefully, this future additional med will create enough competition to drive down the price or a generic becomes available.

      There are no generics of biological products. What you may have is a biosimilar. However, developing a biosimilar is very expensive. It is not like a generic of a non-biological product.

      Genetics probably won’t be available until a drug is 10 years old. This is the way companies do it and how it’s regulated.

      @@kam0406 The patent for Victoza, which is Saxenda for weight loss ends this summer. There are already generics scheduled for next year.

    Just started Ozempic a week ago. Have already lost about 3 lbs. Will see what happens long term. Currently 261, highest weight in my life was 280 a few years back. 266 right before I started Ozempic.

    Saxenda is a game changer! I already lost 8 pounds in 2 weeks! No crazy symptoms just feel very tired sometimes and thirsty. Other than that I feel great! I need to lose about 50 lbs

    Thank you for this wishful thinking. I am a newly diagnosed type 2 diabetes patient with PCOS & hx of kidney cancer...My prescription plan 100% does not cover Wegovy rx'd by my doctor, and the manufacturer coupon of $500 off of the $1,300 cost; still is not do-able for my current budget. I will be on the look out for FDA approval of the trizepatide. It seems to be life changing! Thank you again & blessings to you.

    I’ve been on Ozempic for weight loss for 5 weeks just did my first .5mg last Monday and I’ve lost 10 pounds, but unfortunately since I don’t have diabetes my ins won’t cover it so now my dr wants me to try wegovy since they aren’t going to cover anything, it’s messed up that ins will cover bariatric surgery but not weight loss meds

    I think that's great, Wegovy is prohibitively expensive right now and the more competition in the market there is, the more accessible these drugs will be

      Unfortunately, we likely won't see that play out as we would expect from a competition perspective. Drug marketing/selling is a bit different compared to say selling cars. The only time we will see prices come down is when generics start to come to market.

    Ozempic was horrible for me. In fact none of these have been worth the side effects, the worst being heartburn that ulcerated my esophagus. Ozempic raised my sugar from 140 to 335 in 72 hours but the worst part was that my feet swelled to the point where I couldn't even get slippers on. I changed my primary and the next one bullied me into trying it again, yep same result! Tried mounjaro and the side effects were again horrible. Spent a month on my😊 couch throwing up. Lost 7 lbs and my sugar dropped from 305 to 140. Couldn't renew because my BCBS refused to pay for it. Now I'm on nothing😢 maybe I should try keto? I'm only 20lbs overweight.

    Hi Dr new to your channel. I was on Wegovy for 6 months. Lost about 20 lbs on lower doses but didn’t feel well the whole time. After I went up to 1.0 and beyond I started major joint pain especially in my knees and nerve pinging similar to what I would say neuropathy would feel like on my legs and arms. I asked my dr if this was normal and since it was new to her she didn’t have a clue! And the nausea was unbearable. I gave up. I need to lose about 30 lbs so I can get knee replacement. Also I need to mention I have gained the weight back and more! I have tried them all over the years but can’t tolerate the speedy feeling. Any thoughts on this?

    The current highest dose Tirzepatide already seems to be closing in on highest dose Wegovy, although not quite there. Really hope higher dose is safe as well with little side effects.

      Exactly. I suspect side effects will be the main concern for higher doses.

      Tirzepatide is likely better than Wegovy. Tirzepatide achieved 13% weight loss in diabetic patients, Wegovy achieved 15% in non-diabetic patients. Non-diabetes patients lose more weight on GLP-1 agonists.
      Tirzepatide in trials with non-diabetic patients explicitly for weight loss. Given that it achieved 13% in diabetic patients, it will likely achieve higher weight loss in non-diabetic patients.

    Great info as always, thanks Dr. Dan! In the study, what kind of side effects were patients seeing? I'm ok Ozempic and having incredible results, luckily only side effect for me has been occasional heartburn.

      Primarily GI in nature. Nausea, heartburn, vomiting, constipation, etc. Very similar to what people experience with Ozempic.

    I am on week 7 of Ovempic . No changes in my blood sugars at all. 16-17 most days. I have stomach pain, bloating, nausea daily. I’m not liking it much. I know my appetite has become smaller. Eating less. But no change in weight either. When you walk around each day feeling like shit, it’s difficult to be positive that this is a drug that can help you. If these feeling continue , I’ll be discontinuing this injection.

      Hi i stop ozemptic after one month on it...having sideeffects lots stomach pain swolleness alergies in my eyes...went to emergency...n was having acute Pancreatitis...horrible pain...check ur self...n stop.taking the injection...realy sick. 🙏✌🌹

      I stopped using Ozempic per physicians orders after experiencing extreme 24 pancreatic pain. Took about 2 weeks to feel normal again. I did however lose 25 lbs being on it. Might try again in 6 months. But for now happier having back to my normal pain free, less bloating, no nausea in my life!

    Probably going to be more effective as the higher dose of wegovy lol. As a Dr you know we have to see it first because we do not know what happens at the higher dose. This is just a it should post….Symptoms on a higher dose could be so unbearable that it’s not able to be used.

    I’ve been on Rybelsus for 3 months first month on 3mg, 2nd month on 7mg and 3 month on 14 mg for pre diabetes not loosing any weight and work out 4 days a week. Sugar levels are great but would like to get the weight loss benefit

    Always have the scoots on ozempic hope this isn't the weight management only started the dose of 0.50 I have a fatty liver, A1C 5.9 so on it for that but I also want to lose weight.

    What about the price? Ozempic cost me $529.00+ (with Part D coverage) for a 3 month supply. I lost 22 lbs. in four months. However, it did nothing for my Glucose level. I had to stop taking Ozempic due to the price.

    I get semaglutide for $200 a month. It's the same thing, not sure why people keep going for the name brand. Saves me more than $200 on groceries too cuz I can't eat. The nausea and fatigue is killing me though. I may eventually just choose to be chubby and happy

    My Doc just reviewed Wegovy with me on my last routine visit and I can’t wait to try it! Over the years I’ve tried every diet on the market and even had Lap band surgery. Nothing has worked long term for me. This sounds very promising!

    I’ve been on Ozpemic for diabetes and loss 50 lbs. my daughter is consider Wegovey. My questions are if anyone knows about insurance. With insurance and a discount from Ozpemic I can get it for 24.99. How does this compare to Wegovey or Triceptic (sorry forgot how it’s spelled)? Thx

    To combat massive fatigue can you take phentermine or are there contraindications in doing so, I wonder? I think the nausea patch might be a good thing as well. Seems many prefer taking the shot at bedtime because of side affects associated. Thoughts anyone?...

      I was thinking yes, I did a small dose of Semaglutide but it didn't work for me; not strong enough; however I was very tired. I'll be trying WeGovy this Friday. I have taken phentermine and it's great for giving you energy.

    I'm so over these healthcare companies. My health insurance company doesn't cover Wegovy, but will cover weight loss surgery. This makes no since health and money wise. $800 a month for Wegovy.

    If I change my diet and add more exercise, can i stop the drug and not regain the weight? I plan to do all 3 but I'm so afraid I'll regain the weight

    If these drugs all started out being ways to lower A1c for people with type-2 diabetes, then for those people who don’t have diabetes, but who are obese, what does taking this higher dose of 2.4 mg do to their A1c?

      The cool thing about these medications is that their mechanism in blood sugar lowering actually shuts off when your blood sugars are in a normal range!

    When patient loses 6.7% bodyweight with Semaglutide 1mg after 40 weeks, is that 40 weeks on full dose after previous titration to 1mg? Does the % loss typically translate to higher weight patients? (not 94 kg, but maybe 138 kg, in which case he would lose about 9kg)
    By the way, 138 kg is my dad’s weight when he started Ozempic. His diabetologist eventually aims for 94kg bodyweight😀 Not sure if it’s a strategy to pump dad up so he achieves “disappointing” 110 kg or something😅

      Great questions. You are right. 40 weeks at full dose however, many patients also lost weight during the run up to 1mg once weekly. Yes it does translate - on average whether the patient was 94kg or 138kg they all lost on average 6.7%. Which would translate into the respective weight amounts in each group!

      I saw someone say $1200 on another channel if you don't have insurance its expensive and insurance will only cover it if you are diabetic.

      This is impossible to say everyone loses weight at different rates. The study was over 1 year.

    Hello could you please tell me if Victosa and Jardience are included for weight lose? I take both for Diabetes type 2. I have lost weight over the past year but i really have a bad sugar addiction.

      Victoza is Saxenda just at a lower dose. Some people do lost weight on Jardiance as well

    5.8% A1c?!? That's pretty much non-diabetic as 5.7% is the normal upper limit.

      Depends on the definition, and guidelines you look at. 5.7%-6.5% could be considered pre-diabetes.
