Social Determinants of Health Accelerator Plan

In 2023, the Peoria City/County Health Department received a CDC grant to gather resident input on key projects that could be quickly done, or accelerated, within the 61605 zip code to improve quality of life.
We talked directly with residents, gathered survey data from residents and community stakeholders, and met with local government departments to gain a full understanding of the area's needs. The plan was constructed with a Social Determinants of Health lens. In other words, how healthy a person is and their quality of life is determined by many factors.
The result was an Accelerator Plan. Our team is now presenting the plan to government bodies and community members.
