    What can be done? More full-time staff urologists at VA hospitals and residencies in prostate cancer to train them. Earlier PSA tracking done annually to support early detection. MRI or PSMA PET scans done when PSA crosses 2.0 levels or PSA velocity increases above normal thresholds, followed by focal fusion biopsies, if lesions are detected. Genomic testing to identify higher risk type of lesions that progress quicker. Faster progression to surgical treatment when biopsy, CT and bone scans indicate prostate confinement of cancer. Employment of state of the art focal treatment methodologies, already deployed in the civilian community, where appropriate. Cancer growth doesn’t wait while red tape is being processed or limited resources are rationed out. Timely diagnosis and treatment is of the essence, before metastasis can progress. Early treatment is cheaper and more successful than treatment delayed. When resources are deployed early, less are needed.

      i got out ov the VA system when i learned that they dont have a lot of things . no pet mri capabilities , genomic testing , or any focal therapies to include brachy . they either chop you or burn you with external beam . i intended to get out of their system before i get any older anyway . they are too authoritarian and powerful . they could abduct you and throw you into one of their nursing homes particularily if you dont have family to advocate for you . my local hospital signed me up for medicaid until im 65 and shift to medicare . ( im 64 now ) . this hospital has all of the most current tx options available to them .

      @@searcymasonry My VA hospital in Cleveland has multi-parametric 3T MRI, Axumin, Technitium99, and PSMA-PET scans, genomic testing via Salt Lake City which tests all the VA samples, brachytherapy, SBRT, IMRT, and IGRT with a Varian TruBeam. The surgeon comes over from Cleveland Clinic to do robotic prostatectomy. I couldn't do any better at Cleveland Clinic, a center of excellence for prostate cancer.

      @@eddiegardner8232 my va doc ( roudebush / indy ) wrote right in my files that brachy wasnt an option at any va . so she lied , im not surprised as they were simply trying to strongarm me towards surgery ( with no nerve sparing btw ) . im in IU health system now and im kinda interested in hifu ( which IU invented ) . im favorable 3 + 4 intermediate and it appears that hifu is suggested for that grade of dx . roudebush gave me two choices , chop or burn . they had no intention of even doing any imaging . my gut told me to get the hell out of there .

      I was Gleason 4+5, no extraprostatic extensions, so I did robotic prostatectomy, bladder neck sparing, bilateral nerve sparing, negative margins. One positive node on ePLND, so I did pelvic node IMRT radiation after surgery. On abi-pre now with ADT (Lupron). Testosterone and PSA currently undetectable. Best wishes for your successful treatment. PS. Didn’t cost me a dime, including the genetic testing, which was negative for all mutations, so no PARP inhibitor for me.

    I am Vietnam Veteran with advanced prostate cancer. It has been treated as service connected since diagnosed in 2013. Now it's in my bones and VA wants to do genetic testing on me. Will this affect my disability benefits??

    100% total incontenance after robotic now ,after 7 years on a holister catheter. My penis&scrutom have ulcers Daly advise
