Technically by Disney canon Darth Vader’s first red light saber was taken from a Jedi shortly after revenge of the sith he made the crystal bleed and eventually that saber was destroyed and he then built the one we recognize from the films

Darth Maul appeared in the show Star Wars the Clone Wars(the 3D one), he had robotic legs and an apprentice named Savage Oppress. He was killed in a duel with Kenobi on Tatooine.

The reason for General Greivous coughing is because mace windu used force crush on his ribcage lung area and damaged his lungs.
