    SUB CLINICAL HYPOTHYROIDISM
    1. Subclinical hypothyroidism is defined biochemically as a normal
    serum free T4 concentration in the presence of an
    elevated serum TSH concentration.
    2. It may occur in the presence or absence of mild symptoms of hypothyroidism.
    3. A substantial proportion of patients with subclinical hypothyroidism eventually develop overt hypothyroidism.
    4. It may be associated with an increased risk of cardiovascular disease , particularly when the serum TSH concentration is above 10 mU/L.
    5.Management is entirely based on the clinical age group and associated clinical conditions.
    SUBCLINICAL HYPERTHYROIDISM
    1.Subclinical hyperthyroidism is defined biochemically by a low serum TSH concentration but normal serum free T4 and T3 concentrations.
    2.Patients with subclinical hyperthyroidism typically have few or no symptoms of hyperthyroidism.
    3.The most common causes of subclinical hyperthyroidism are treatment with T4 and autonomously functioning thyroid adenomas and multinodular goiters.
    4.Subclinical hyperthyroidism is associated with an increased risk of atrial fibrillation and, primarily in postmenopausal women, a decrease in bone mineral density.

    Hi, my blood test came back with 2.27 mIU?L (range 0.27 - 4.3mIU/L) and T4 borderline low with 62.2 nmol/L (range 66.0 -181.0nmol/L). I have some of the symptoms of hypothyroidism brain fog, thin hair, loss of outer eye brows and especially general fatigue. Is it worth getting levothyroxine for an underactive thyroid?

      I believe the 2.27 value is that of TSH . If so , It indicates a normally functioning thyroid. Levothyroixine is of no help in the presence of normal functioning thyroid.
      Free T4 is the active form present in the circulation. Testing for Free T4 can be helpful in your situation as Total T4 falls short in diagnosing the thyroxine status. In your scenario the Free T4 might turn out to be normal.
      Most probably speak with your doctor regarding the cause of your symptoms which most likely is not due to thyroid.

      @@interpretyourlabtests7734 Thank you so much for your help. I have just checked my blood test results again and noticed free T4 falls within the normal range. I'll see my GP beginning of February. Hopefully she can help me find the cause of my symptoms. I am going through menopause and take a prescription for HRT so that might have something to do with it.

    Total T4 = 11.2 ug/dl (u=microgram/dl)
    TSH = 1.90 uIU/ml
    Reference ranges: (t3=60-200; t4=4.5-12; tsh = 0.35-4.94)

    Hello, can you help me with these results. I have empty selle syndrom which causes Hypogonagism, adrenal insiffiency and Hypothiroidism. For the last 2 my new endo is not sure that i have those and told me that i possibly been Cortef and Thyroid for nothing for over 2 years. I inject with testosterone twice a week and take 90mg of Thyroid and 30 to mg of Cortef plus high blood preassure meds... Thank's for your help

      FT mean Free thyroxine .
      Levels are indicative of subclinical hypothyroidism . However mentioning he units is even more helpful

    I was taking 100mcg of Synthroid 6 days a week for the last 6 years.My TSH recently was 0.05L and the Doctor reduced it to 75mcg every day. Now my current test came back at 0.19L T4free 1.3 & T3,total 92. Is the current TSH borderline normal? Do you think the dosage should be reduced based on these current results? I always thought t3&t4 were the important tests.

    Starting from May I am using thyroid medicine and nothing is changed becuase my doctor is only test TSH AND T4. I dont understand why she is ignoring t3 test.

      TSH is the only thing which matters while deciding the treatment dose especially in hypothyroidism . T3 helps in confirmation of hyperthyroidism . Most likely u may be having hypothyroidism and hence TSH and T4 are enough

    Be very careful how the ranges uses different units. They confuse you. Some Lab gives T4 @ ng/do. Look at what this youtube tells you. Micrograms per dl. What is that mea? Not a good explanation.

      TSH levels seems to be fine at this point . fT4 seems to be in he higher end (as per the common ref ranges ) . If it is little elevated when u compare with the lab range. It is recommended to monitor TSH atleast after 6 weeks again

    Hi! I am going through the preliminary diagnosis and am told I have Hypothyroidism. (Male, 39yrs)
    - My TSH basal ulU/ml is shown as: 35,18+ (its range is shown as 0,27-4.2)
    - FT3 pg/ml is 2,20 (range is 2 - 4.4)
    - Ft4 pg/ml is 5,50- (range is 9.3 - 17)
    My doctor simply prescribes Thyroxin 50mg but no further explanation as to the causes, or any other guidelines.
    Could someone help me to interpret the high value of 35,18 ?
    I only coincidently went in for a blood work and did not experience anything major fatigue or such and am afraid to start hormones for the fear that this will become a lifelong thing.

      Something seems to be not right with the units of ft3 and ft4 .please check that .
      Pts who have a high TSH value (> 10 uIU/mL ) needs to be treated irrespective of free t4 levels
      If free t4 is low it is clear case of hypothyroidism which needs treatment .
      If the Free t4 is normal , it's a case of subclinical hypothyroidism . These persons have very high chances of developing hypothyroidism .

    My TSH value was 18.5 and I was placed on levothyroxine 2 weeks ago . I checked my TSH again today and it's 17 . Do I have hypothyroidism ?

      Increase in the TSH together with symptoms is a clear indication of hypothyroidism .
      However retesting of the TSH should be done only after 6 weeks. Reaching a steady state of hormones in the body takes 6 weeks ( As per the half life esp,T4 ) . Hence retesting is not recommended before the stipulated time

      If the upper reference range for TSH is 5.5 and lower reference range for T4 is 0.8 , ur results are indicative of hypothyroidism .
