This is a great tutorial! I have been a nurse for many years and I am a nursing instructor. Documentation is so important because it tells a story about the patient. I review charts and the nursing documentation in a lot of cases is extremely poor. Also, asking questions is very IMPORTANT when you do not know the answer. It's patient safety and your nursing license. All nurses should ask questions when they don't know the answer because healthcare is forever changing. Keep up the good work and you are going to be fine. It sounds like you are on the right track! Congratulations and welcome to nursing!
