    I am a conundrum for pulmonologist. Emphysema diagnosed after sudden short rss of breath of a life long athletic enthusiast and strict non smoker. My mother had COPD diagnosed very late in life as life long nonsmoker but couch potato. and obese diabetic as well. I tested negative for Alpha one and Dr is resubmitting test . I have very poor spirometry results at around 30% after taking multiple inhalers daily. I began at 20% and focused more on gastric reflux , and asthma a couple of years ago . My reflux improved but not perfect altho I stopped PPIs over a year ago due to noticeable side effects . This all began suddenly with menopause as well. I cannot believe what has happened to me and its highly depressing as well as unbelievable . I have gained almost 20 lbs ONLY in midsection even though I continue to exercise as much as I can . I luckily was very slim and tall so its not horrid . It all is very depressing really.

      Oh, Btw , my cat scan was the tell all of my present diagnosis .... read as severe pulmonary emphysema and then it all came together one year ago.
      So I guess after presenting as the epitome of health all of my prior life I must accept early death and suffering like a careless lazy patient - so not fair !

      Also I take Bio identical hormones and it hasn’t helped much except extreme symptoms . I guess I am screwed as they say ... Well I am pissed off at God bc he is very unfair . The God of the Old Testament is harsh, judgemental, and required blood sacrifices - so I only believe in the purity and love of Jesus ... HIS TRUE FATHER IS ABOVE HUMAN COMPREHENSION . Maybe I will have his blessing one day because I never deserved this

Does it believe that the infiltration of smoke is a pathogen that requires proteases to break down and dispel, and a by-product of this is the breakdown of elastin within the alveoli?

    Hi! Thank you for your videos. Could you explain why someone with emphysema wouldn't have respiratory acidosis? I thought that since they are hanging on to PCO2 that they will be in acidosis. Thank you!

      Early on: they are hyperventilating,
      But later: hypoventilation leading to respiratory acidosis.

    Can u help me please ? If the alveoli filled with air so gas exchange will occurs correctly and no hypoxia but later on the alveoli will be destroyed and further on no gas exchange am i right? Destroyed by what?

    At 7:12 u said air trapping causes acidosis. and at 17:34 you wrote emphysema causes respiratory alkalosis.
    So finally does emphysema causes acidosis and alkalosis?
    Im confused now.
    And if there is so much air trapping, why does hypoxemia occur?
    On the slide at 17:24 you said hypoxemia is because it takes time to destroy the respiratory unit. what does that mean?

    Can someone explain to me? : emphysema pt are grandpa's socks --> alveoli cannot recoil --> retains air, air stacks on air ---> cannot blow off air --> sounds like they cannot blow off co2 to me --> sounds like co2 accumulates to me ---> sounds like resp. acidosis to me. --------why do they have low PaCO2 after all?????

      found this from GOOGLE. Need an explanation. Still confusing.. "In the mild and moderate stages, the PaO2 and the PaCO2 measurements may remain normal or, while the PaO2 stays normal, the PaCO2 can be decreased (respiratory alkalosis). In the moderately severe and severe forms of emphysema, the patient is likely to be hypoxemic and hypercarbic (respiratory acidosis)."
