Case 78: PCI Manual - Calcium vs thrombus

  • An elderly patient presented with NSTEMI and was found to have a filling defect in a heavily calcified circumflex artery as well as a severe proximal LAD lesion. After being turned down for coronary artery bypass graft surgery he was referred for PCI. OCT showed that the filling defect was thrombus that was treated with the combination of a Spider filter and thrombectomy with an Export catheter followed by stenting with OCT optimization with a nice result. The LAD was subsequently treated with orbital atherectomy and stenting also with a nice result.

    Great case. were vessel measurements distal to the thrombus using OCT not possible? maybe that would have helped using only one larger stent, instead of 2.
    What was the size of the Aspiration catheter (7F or 6F)?
    your broadcasts are really priceless please keep posting these great cases
