  • July 6, 1988, Piper Oilfield, North Sea: As shifts changed and the night crew aboard Piper Alpha assumed duties for the evening, one of the platforms two condensate pumps failed. The crew worked to resolve the issue before platform production was affected. But unknown to the night shift, the failure occurred just hours after a critical pressure safety valve had just been removed from the other condensate pump system and was temporarily replaced with a hand-tightened blind flange. As the night crew turned on the alternate condensate pump system, the blind flange failed under the high pressure, resulting in a chain reaction of explosions and failures across Piper Alpha that killed 167 workers in the deadliest offshore oil industry disaster to date.

That the company wasn't strung up, for not constructing adequate equipment, to indirectly cause the death of anyone inside the control room.. is gut wrenching to learn.
The chain of unfortunate and accidental events, that contributed to further the disaster. That is tragic

It also reminds me OXY Bangladesh blowout 1997 where I was physically present at our first wellsite.

Thanks for the info. May you please give reasons and root reasons which cause the accident? I would highly appreciate it if spoken in safety terminology and methodology

On June 14, 1997, a destructive gas well blew out at the Magurchara gas field in Kamalganj, Moulvibazar district. I worked at that location prior to the blowout
I was on contaract to Oxy for 2 years, they also had a close shave in Sylhet around 1998 with a gas well

There is no “e” in Tartan. A production shutdown also does not cause a problem to the “entire power supply” of the platform. Two completely different things.
That makes it sound almost justified not to shut down production.

I am From the Petro-Chemical Industry in Australia Manufacturing Iso-Butanol... N Iso-butanol and Iso-Octanol.. from Synthetic gas 50% Hydrogen/Carbon Monoxide ... Propylene and Heptane.. we had an Incredible Clearance to work System in place BUT it failed on Saturday Morning at Shift change resulting in Hot work being carried out in the wrong Place (Oxy-Cutter) A tank that had a Nitrogen sweep on it was turned of for some reason resulting in Oxygen contamination in the gas space ... BOOM.... 20.000 lts of an organic cocktail went up , tore the Tank off its foundation .. resulting in 5 deaths and many injuries during an Annual shutdown I was driving back from Queensland when i heard it on the radio ... Me !!! I was a Shift Manager for ICI Australia... thanks for reading .... foot note we use to use Piper Alfa in our Safety Training video's...
