The importance of nocturnal blood pressure

  • We recognise that isolated blood pressure recordings are entirely useless. They can not be used to make a diagnosis. They can not be used to predict prognosis and they can not be used to monitor a patient’s response to treatment. They are a waste of time.
    The best way to get a good understanding of blood pressure is to use something called ambulatory blood pressure monitoring. Here a blood pressure machine is strapped to the patient for 24 hours and the machine will automatically measure the blood pressure twice every hour during waking hours and once every hour at night and then calculate 3 sets of average from all the readings. It will give you a 24 hour average, a day-time average and a night time average. All research studies on the subject of blood pressure have found that ambulatory blood pressure monitoring is the most reliable non-invasive method for an accurate assessment of blood pressure. The results are reproducible because all the variability tends to get averaged out. An ambulatory blood pressure monitor can be helpful to give you an accurate representation of blood pressure numbers, it is more accurate in terms of predicting prognosis and it can reliably assess response to treatment.

    I’m usually very sceptical of CZcams videos on health, even some where the creators are , or purport to be, qualified medical practitioners. But this series, from a consultant cardiologist giving us all the benefit of his advice free, with no dietary fad to peddle or product to sell, is worth its weight in gold. This is what the internet is for. Ideally it would on free to view TV, as a series of shorts, perhaps with similar episodes on other aspects of health.

    It's great you bring this subject up. BP is not a physical constant like gravitational pull. Actually, a
    new German study determined that higher BP lowers morbidity of older people (European heart journal). Who of the huge world population has actually BP checks? 1% ?
    Still, many unknowns.

    I have fear of having my bp taken, I now have fear of the machine, the noise and while the cuff tightens, I can feel adrenaline running through my body, I have to keep taking it, the lowest I got was 132 over 82 but I was still anxious, I feel I can’t get a Normal reading now I have this fear, it’s not just at the doctors I’m now scared to do it myself.

      I have that really badly too. I’m just now calming down abt it. My doctor was trying to double my bp meds and I was telling her I didn’t believe it was that high usually because I swell a lot when it’s truly high and I wasn’t. I was right. When I relaxed and cussed myself out the numbers came down to normal. I still get a higher number the first time so I sit still and wait several minutes for the second reading. You will get a baseline eventually but obsessing abt it drives it way up. In her office my bp was 198/100 at home it was 126/70. Anxiety will drive it crazy high don’t let anyone tell you differently. Still we must watch our stress, diets and drink water and exercise. Hugs♥️

      @@kapiljbp9129 I think that's a normal effect of the BP- taking process, i.e. blocking blood flow and then letting it go slowly. It happens to me and I have low BP.

      @York Cardiology Hi sir, i am 33 and chest pain and pain in arm but its gone now. i had skipping beats and heart palpation which is also gone. but sometimes when i wake up from sleep my heart beat gets faster . and sometimes if i eat salt or sugar my heart rate gets faster for 5 minutes and then slow down again. i dont understand why my heart beat gets faster sometimes and normal most of the time. i dont know if its because of stomach or heart problem.

    Dear I agree with you 100% whatever you said. The only problem is that the doctors don’t agree with these kind of theories and don’t want to act and treat their patients as you said. They tell you to take the medicines as prescribed or change the doctor. Thank you very much for showing us the reality of the modern treatments. I believe in whatever you said and try to apply on me as much possible. A time will come that I might be without a doctor in future.

    Thank you. I'm glad I have this information, now. I wish I had it prior to my Dad's passing. I believe his health or lack there of was the result of medical mismanagement. He was a veteran. My Dad deserved much better from his doctors, but they pushed pills. They were licensed drug dealers, in my opinion - not healers.

      My Dad had very high pulse level (tachycardia). I found out that his doctor was giving him six doses of Albuterol a day. I told the Nurse practitioner that he had never used Albuterol before and probably needed it once a day, if at all. She said, "I'll check on that."
      Bless her, she did, and my dad had 5 doses of Albuterol removed. He lived for three more years without weird rythyms until 92.

    I wore an ambulatory bp monitor..I was so stressed by it, my bp was much higher while I was awake...and lower while asleep.. My doctor implied that it was a bad thing to have low bp at night. I refused to go on meds b/c my bp was normal when taken at home (we have a bp machine).

    Thank you thank you! U r the first dr that understands what i have been going thru BP NORMAL during the day n high at sleep. Drs including cardiologist dont understand why. Ive been on cpap and it did not help. I bought a bed that elevates
    Your head. No help ! I took it upon myself to start taking my BP med at night it has solved my dilemma. Thank u for showing me im not weird it does happen to others. God bless

    Wow! You are spot on! You summed it up perfectly. I absolutely hate dealing with the cardiology dept where i go. Lots of big egos and pill pushers who dismiss your concerns. I deal with other departments in the medical system and rheumatology and cardiology are the worst.

    My blood pressure is always high when I know I’m going to see a doctor. It’s starts a few days before, while I’m there and I feel fine when I’m at home. I’m stressed because anything I mention he’s ready to prescribe a pill. Never heard him say anything about changing diet, exercise, anything, just pills. So both my husband and I are doing what we can to better our health naturally ,through diet, walking, vitamins, sleep, whatever it takes. No body cares more about us than us! And when my time is up, it’s up! When the Good Lord says so❤️

    I noticed that my blood pressure has gone up since my beloved brother passed away suddenly and unexpectedly eight months ago. I find that if I make concerted efforts many times during the day to stop and take time to deep breathe, it helps right away to bring my BP down.
    P.S. Hearing your calm voice and good advice also helps.

    Another excellent video. This time some of its content was expressed in bolder language. The word 'profit' was used and its relevance to current guidelines was discussed. I call that excellent and enlightened medicine. The words 'majestic integrity' also come to mind. It is a great privilege to hear a man of such appropriate learning and abundant experience speak of these things with such judgment and balance.

    I would really appreciate a video on blood pressure differences between both arms and your arms and legs.
    I would demand that ambulatory blood pressure monitoring before I would accept drug treatment. In the US, you have to arm yourself with knowledge or your doctor’s health insurance records algorithm loves to over prescribe and the doctor has to override that in order to treat you individually. Not all are willing to do this, especially because their records can be searched electronically for their compliance with insurance “guidelines” and their malpractice insurance rates raised accordingly-making it very costly to treat you.

    Excellent site! I have been treated for hypertension for 30 years.....am 66 now.....and have never known why. Take Lisinopril and Lecanadipine every day.
    I am not over weight. Eat healthy. Walk daily. Non smoker.
    Guess I have some homework to do.

    Very very well explained Dr Gupta , I am one of those non dippers where I have normal BP in the morning but it does not dip in the night but it increases but then since ones blood pressure is normal in the day time giving a blood pressure medication could put you into low blood in the morning and that is not desired either . So it’s a kind of a conundrum . But having seen your video and hearing your honest comments I will certainly pursue this with my health team . Thanks so much.
