Solution:
Try to unplug a few USB cables from the PC and than try to boot from the bootable USB flash drive

P.S - I used Universal USB installer instead of Rufus, because Rufus doesn't let you change format NTFS to FAT32

Worked for me! I removed a cheap usb hub connected at the back of my cpu tower and the error is gone. I was having this error whenever i wanted to clean format my pc for a long long time and didnt know the solution is quite simple lol should have done this before omg
this error can also be caused by dust on the USB port or even the USB port is faulty

Good thought, I still get this with the drive hooked directly in with just a mouse and keyboard on every machine using flash drives produced by Rufus. Still looking for solutions :(

Hay q desconectar todos lo q sea usb y usar teclado y mouse ps2, además bootear primero con plop boot manager 5, después elegir la unidad usb donde está el instalador de windows
for those who have an old BIOS 😂😂, yes, now those who have at least a legacy BIOS from 2005+ do not suffer from this as it differentiates each connected USB device and detects whether it is bootable or not

hey man im trying to clean install windows 10 and after i press any key it keeps showing that windows in the middle and nothing happens i wonder if it's stuck or it really takes alot till shows up the installing process?
It requires some time, but it also might have some difficulties... Are you starting the installation from a scratched DVD, or from a slower USB 2.0 drive or from the hard drive?

You have to disconnect all that is usb and use ps2 keyboard and mouse, also boot first with plop boot manager 5, then choose the usb drive where the windows installer is

Hey buddy, i got a new HDD 1 TB, I inserted the flash drive, it booted and then setup for Windows 7 was also done. Now after the setup was done, i get the error "A disk read error occurred". I only have the USB receiver of the mouse inserted. Plus, i have also tried without any other device in any usb port. Still the same issue, any help?
- enable everything USB related in the BIOS
- make sure motherboard drivers are installed under Windows
- try to download and install a newer Windows also (keep the existing one) and try it under that one
- temporarily unplug some devices inside and outside the PC, for example another hard drive, or cd/dvd drive, or even the mouse

first i went to advanced bios settings then my primary device was CD/DVD and my sata hardrive was secondary. So i changed the primary to HDD and secondary to CD/DVD
In my case I was having a problem with Rufus. I used "Universal Usb Installer" and it worked.
My motherboard is M5A78L-M LX/BR

Tipo ele criou o pendrive de boot com um programa chamado rufus, a solução foi usar esse outro programa, universal usb installer.

Can you help me? When i use flash drive with windows 10 its working, but when i using it with windows 7 or windows 8.1 its not working and there is disk error, ctrl alt delete and can't install windows. Pls help mee
I'm not sure but maybe there is protection in the BIOS against older operating systems, I can't install Windows 7 either, 8.1 is always complaining about some incompatibility...

Me volvi loco buscando solucionar este error. A mi se me resolvió usando un booteador que permita usar formato fat. Lo hacia con el rufus y no funcionaba porque ese programa usa solo ntfs, o a mi solo me dejaba usar ese y no me dejaba cambiar, estaba instalando windows 7. Usen instalador universal que trabaja con linux.
y si es una laptop como le quito el mause

- not even a wireless mouse?
- check if the ISO is really bootable (try VirtualPC and boot from the USB)
- make the USB bootable again using another ISO image
